摘  要:目的:分析羟考酮对全凭静脉麻醉苏醒期血流动力学及机体免疫功能的影响。方法:选取2015年3月-2017年3月在笔者所在医院就诊的72例全凭静脉麻醉患者为研究对象,随机分为两组,各36例,术前10 min对照组给予芬太尼,观察组给予羟考酮,比较两组苏醒期血流动力学和机体免疫功能。结果:两组患者睁眼时间、自主呼吸恢复时间及拔管时间比较差异均无统计学意义(P>0.05),观察组躁动程度评分明显低于对照组,差异有统计学意义(P<0.05);血流动力学结果显示,两组患者t_1、t_2时的MAP、HR均明显高于t0时,差异有统计学意义(P<0.05),两组t0时MAP、HR比较差异均无统计学意义(P>0.05),观察组t_1、t_2时的MAP、HR均明显低于对照组,差异有统计学意义(P<0.05)。免疫功能检测显示,两组患者麻醉后的CD8^+均明显升高,CD4^+、CD4^+/CD8^+均明显降低,且观察组CD8^+明显低于对照组,CD4^+、CD4^+/CD8^+明显高于对照组,差异均有统计学意义(P<0.05)。结论:羟考酮对全凭静脉麻醉苏醒期躁动有改善效果,且对血流动力学有一定的稳定效果,可有效改善患者麻醉后的机体免疫功能紊乱状态,值得推广。ObjectiveTo analyze the effect of Oxycodone on hemodynamics and immune function during anesthesia in awake period.Method:From March 2015 to March 2017,72 cases of total intravenous anesthesia in our hospital were selected and randomly divided into two groups with 36 cases each.The control group was given Fentanyl 10 minutes before operation,the observation group was given Oxycodone.The hemodynamics and immune function of the two groups were compared.Result: There were no significant differences in the open eye time,spontaneous breathing recovery time and extubation time of the two groups(P〉0.05),the degree of agitation in the observation group was significantly lower than that in the control group(P〈0.05);the results of hemodynamic showed that the MAP and HR at t1 and t2 of two groups were significantly higher than those at t0(P〈0.05),there were no significant differences in MAP and HR between the two groups at t0(P〉0.05),MAP and HR of the observation group at t1 and t2 were significantly lower than those of the control group(P〈0.05).The detection of immune function showed that the levels of CD8+ of the two groups increased significantly after anesthesia,and CD4+ and CD4+/CD8+ were significantly decreased.The CD8+ in the observation group was significantly lower than that in the control group,CD4+ and CD4+/CD8+ were significantly higher than those in the control group(P〈0.05).Conclusion:Oxycodone on total intravenous anesthesia has improvement effect,and has certain effect on hemodynamic stability,can effectively improve the immune function of patients after anesthesia state of disorder,is worthy of promotion.
