摘 要:目的观察青藤碱电致孔透皮给药治疗老年膝骨性关节炎急性发作的疗效及对膝关节滑膜液中炎性因子的影响。方法将70例老年膝骨性关节炎急性发作患者随机分为2组,对照组35例给予布洛芬缓释胶囊联合盐酸氨基葡萄糖片口服,观察组35例在对照组治疗基础上给予LHJ-X1型原道电致孔电子脉冲治疗仪配合青藤碱注射液透皮给药治疗,2组均以10 d为1个疗程,治疗2个疗程。观察2组治疗前后关节VAS疼痛评分、关节WOMAC评分及膝关节滑膜液中一氧化氮(NO)、超氧化物歧化酶(SOD)、丙二醛(MDA)及肿瘤坏死因子-α(TNF-α)、前列腺素E_2(PGE_2)、基质金属蛋白酶-3(MMP-3)水平变化情况,统计2组不良反应发生情况。结果治疗后,2组VAS评分、各项WOMAC评分和膝关节滑膜液中NO、MDA、TNF-α、PGE_2、MMP-3水平均明显低于治疗前(P均<0.05),膝关节滑膜液SOD水平均明显高于治疗前(P均<0.05),且观察组各项指标改善程度均显著优于对照组(P均<0.05)。2组治疗期间均未见明显不良反应发生。结论青藤碱注射液通过电致孔透皮给药治疗老年膝骨性关节炎急性发作可迅速改善患者关节疼痛、僵硬等症状,提高膝关节功能,可能与调节氧自由基代谢、减轻关节炎症反应有关。Objective It is to observe the effect of sinomenine electroporation and transdermal drug administration on a- cute exacerbation of knee osteoarthritis in aged patients and its effect on inflammatory factors in synovial fluid. Methods 70 aged patients with acute exacerbation of knee osteoarthritis were randomly divided into observation group and control group, 35 cases in each group. The patients in the control group were treated with oral ibuprofen sustained-release capsules and aminoglucose tablets. On the basis of the above treatment, the patients in the observation group were treated with transder- mal administration of LHJ-X1 type electrosurgical ion pulse therapy instrument and sinomenine injection. Continuous treat- ment 10 times as a course of treatment, both groups were treated for 2 courses. The changes of VAS pain, WOMAC score, and the levels of NO, SOD, MDA and TNF-α, PGE2 and MMP-3 in the synovial fluid were observed before and after treat- ment in both groups, the occurrences of sided effects were recorded. Results After treatment, VAS scores, WOMAC scores and the levels of NO, MDA, TNF-α, PGE2 and MMP-3 in the joint fluid of the two groups were significantly lower while that of SOD in the joint fluid was higher than those before treatment ( P 〈 0.05 ) , and the improvements of every indexes a- bove in the observation group were significantly better than those in the control group (P 〈 0.05 ). No significant adverse reactions occurred during the two groups. Conclusion Sinomenine electroporation and transdermal drug administration couldfast improve joint pain and stiffness, improve joint function in the treatment of acute exacerbation of knee osteoarthritis in aged patients, the mechanism may be related to its regulation for oxide free radical reaction and relieving joint inflammatory reaction.
