摘 要:心电图是非侵入性诊断心律失常最常用的工具。然而,常规体表心电图的空间分辨率很低,缺乏敏感性和特异性,且无法获得心脏局部的电活动信息。而侵入性导管标测也存在很多局限性:无法同步标测全心房或全心室,对于血流动力学不稳定的心律失常无法得到充足的时间进行标测等。一种新的无创心电成像技术(electrocardiographic imaging,ECGI)可能对心律失常的诊断和治疗带来便利。本文综述了ECGI的临床研究案例,包括心室再同步化治疗、室性心律失常、房性心律失常和心房颤动等,为新技术的推广打下基础。Electrocardiogram(ECG)is the most commonly used tool to diagnose arrhythmia noninvasively.However,the standard ECG lacks sensitivity and specificity and its spatial resolution is very low.It can't obtain the information about the local electrical activity either.Invasive mapping technique using intra-cardiac catheters also has limitations,for example,unable to map the atria or ventricles simultaneously and no sufficient time to perform especially for patients with arrhythmias combined with hemodynamic instability.A novel noninvasive electrocardiographic imaging(ECGI)may brings convenience to the diagnosis and treatment of arrhythmia.In this review article,we provide a brief description of the ECGI procedure and review examples of its application in important clinical conditions,including cardiac resynchronization therapy(CRT),ventricular arrhythmias,atrial arrhythmias,and atrial fibrillation to lay the foundation for the promotion of new technologies.
