斑点追踪成像技术评估不同年龄健康孕妇妊娠期左室心肌整体长轴应变的变化规律

摘　　要：目的:应用二维斑点追踪成像技术评估不同年龄的健康孕妇在妊娠期间左室心肌整体长轴应变的变化规律。方法:按不同年龄对健康孕妇和未孕女性分组,<35岁分为A组(年轻组),≥35岁分为B组(高龄组)。对未孕女性进行超声心动图检查,对孕妇在妊娠8周、12周、16周、20周、24周、28周、32周、36周时进行超声心动图检查,分析左室整体心肌长轴应变峰值(LVGLS),比较2组女性LVGLS的区别,分析LVGLS与年龄的相关性;分析2组孕妇在妊娠期LVGLS的变化规律。结果:未孕女性A组和B组的LVGLS差异有统计学意义,且LVGLS与年龄呈负相关(r=0.70)。A组孕妇在早孕期LVGLS先增强,中晚孕期逐渐减弱;B组孕妇在整个妊娠期LVGLS均呈逐渐减弱趋势,2组之间的差异在28~36周时最大(P<0.05)。妊娠期间,孕妇LVGLS与年龄呈正相关,中晚孕期两者的相关性较好。结论:未孕女性的LVGLS与年龄呈负相关,妊娠后逐渐呈正相关;年轻孕妇在早孕期LVGLS代偿性增强,高龄孕妇早孕期则无明显代偿性增强,且在妊娠期间LVGLS持续性减弱。Objective：Using two-dimensional speckle tracking imaging（STI）to assess the changes of left ventricular myocardial global long axis strain（LVGLS）in healthy pregnant women of different ages during pregnancy.Method：Healthy pregnant women and nonpregnant women were divided to two groups according to age.Group A（young women）：age35 years;Group B（older women）：age≥35 years.Echocardiography was performed in nonpregnant women and pregnant women（respectively in 8 weeks,12 weeks,16 weeks,20 weeks,24 weeks,28 weeks,32 weeks,36 weeks）,then to analyze LVGLS.Compare the difference of LVGLS between two groups,and analyze the correlation between age and LVGLS;and the changes of LVGLS in two groups of pregnant women during pregnancy were analyzed too.Result：The difference of LVGLS between A group and B group was statistically significant,and the LVGLS was negatively correlated with age in nonpregnant women（r=0.70）.In group A,the LVGLS of pregnant women increased at first,then gradually decreased in middle and late pregnancy.In group B,the LVGLS decreased gradually throughout pregnancy.The difference between the two groups was greatest at 28-36 weeks（P〈0.05）.During pregnancy,LVGLS was positively related to age,and the correlation was good in middle and late pregnancy.Conclusion：The LVGLS was negatively correlated with age in nonpregnant women,and was positively correlated in pregnant women.In early pregnancy,compensatory LVGLS increases in young pregnant women,but there is no significant compensatory increasein elderly gravida,During pregnancy,LVGLS continued to decrease in old pregnant women.
