摘 要:目的探讨淫羊藿总黄酮胶囊(total flavonoids of epimedium capsules,TF)能否预防绝经后骨质疏松症的发生。方法 5月龄Wistar雌性大鼠48只,采用数字表法随机分为4组:假手术组(Sham-operated,Sham)、切除卵巢组(ovariectomized,OVX)、炔雌醇组(ethinylestradiol,EE)和淫羊藿总黄酮胶囊组(TF),每组12只。除Sham组外各组行切除卵巢术建立大鼠绝经模型,TF组灌胃剂量为265 mg/(kg·d),EE组灌胃剂量为200μg/(kg·d),OVX和Sham组仅灌服等体积蒸馏水。两个月后处死所有大鼠,对各组大鼠进行离体骨密度(bone mineral density,BMD)、骨生物力学、血清骨代谢指标检测、Micro-CT扫描以及重要脏器的器官指数计算和病理学观察。结果实验期间,各组大鼠体质量以及重要器官的器官指数差异无统计学意义(P>0.05),病理学观察未见异常变化,但OVX组和TF组的子宫指数低于Sham组,且差异具有统计学意义(P<0.05)。BMD检测结果:OVX组股骨BMD(0.109 g/cm2)低于Sham组(0.128 g/cm2)、EE组(0.125 g/cm2)、TF组(0.123 g/cm2),且差异具有统计学意义(均P<0.01),各组间椎骨变化与股骨呈相同趋势。生物力学结果显示:OVX组股骨和椎骨最大载荷分别为95.9、201.9 N,且显著低于Sham组,而TF组股骨和椎骨最大载荷值分别为122.1、226.6 N,且显著高于OVX组,各组间弹性模量变化与最大载荷变化呈相同趋势。VG染色结果:与Sham组相比,OVX组的骨小梁数目明显减少,分离度增大,与OVX组相比,EE组、TF组骨小梁数目增加,分离度降低。血清骨代谢指标检测结果:OVX组骨钙素(osteocalcin,OC)含量为104.6 ng/m L,低于Sham组(154.5 ng/m L)、EE组(151.7 ng/m L)、TF组(136.9 ng/m L),且差异有统计学意义(均P<0.05);Ⅰ型原胶原N端前肽(propeptide of typeⅠprocollagen,P1NP)含量变化与OC变化趋势一致;OVX组抗酒石酸酸性磷酸-5b(tartrate resistant acid phosphatase 5b,TRACP 5b)含量为1.57 U/L,显著高于Sham组(0.76 U/L)、EE组(0.38 U/L)和TF组(0.95 UObjective To investigate whether total flavonoid of epimedium capsules( TF) can prevent postmenopausal osteoporosis. Methods Forty-eight female Wistar female rats were randomly divided into four groups: sham group( Sham),ovariectomized group( OVX),ethinylestradiol group( EE) and total flavonoid of epimedium capsules( TF),each group of 12. Rats in each group underwent ovariectomy except sham group. The TF group received TF265 mg/( kg·d) and EE group received 200 μg/( kg·d) EE. The OVX and Sham groups were only fed with equal volume of distilled water. All rats were sacrificed two months later. Bone mineral density,bone biomechanics,serum bone metabolism index test,Micro-CT scan and organ index of vital organs were calculated and pathologically observed.Results There were no significant differences in body weight,organ index of vital organs and pathological observation( P〈0. 05),but the uterine index of OVX and TF groups were significantly lower than that of Sham group( P〈0. 05).Femoral BMD( 0. 109 g/cm^2) in OVX group was significantly lower than that in Sham group( 0. 128 g/cm^2),EE group( 0. 125 g/cm^2) and TF group( 0. 123 g/cm^2),the differences were statistically significant( all P 0. 01). Vertebral changes among groups showed the same tendency as the femur. Biomechanical results showed that the maximum load of the femur and vertebrae in OVX group was 95. 9 N and 201. 9 N respectively,which were significantly lower than Sham group,while the maximum load of femur and vertebra in TF group was 122. 1 N and 226. 6 N respectively,which was significantly higher than that in OVX group,the changes of elastic modulus among all groups showed the same tendency with maximum load. Compared with Sham group,the number of bone trabecular in OVX group decreased,but trabecular separation increased.
