摘 要:目的观察布地奈德、异丙托溴铵与沙丁胺醇联合氧气雾化治疗小儿喘息性肺炎的临床效果。方法随机从医院儿科接诊的小儿喘息性肺炎患儿中选取84例纳入研究,以不同治疗方案分为观察组和对照组,每组42例。对照组采用布地奈德和沙丁胺醇雾化吸入治疗,观察组在对照组治疗基础上加用异丙托溴铵雾化吸入治疗,观察2组患儿临床疗效、症状消失时间及治疗前后肺功能指标变化情况(包括呼吸、心率、SaO_2、FEV1、FEV1占预计值%)。结果观察组总有效率为95.24%,高于对照组的80.95%(P<0.05);治疗后,观察组喘息消失时间、咳嗽消失时间及呼吸困难消失时间均短于对照组(P<0.01);治疗后,观察组心率和呼吸降低幅度大于对照组,观察组SaO_2、FEV1、FEV1%水平提高幅度大于对照组(P<0.01或P<0.05)。结论布地奈德、异丙托溴铵与沙丁胺醇联合氧气雾化治疗小儿喘息性肺炎效果确切,有助于促进患儿快速恢复,提高患儿生活质量,可在临床推广应用。Objective To study the clinical effect of budesonide,ipratropium bromide and salbutamol combined with oxygen atomization in the treatment of asthmatic pneumonia in children. Methods 84 cases of pediatric admissions of children with asthmatic pneumonia were randomly selected from our hospital, divided into. According to the different treatment method, these patients were divided into observational group and control group with each group of 42 cases. The control group were trea- ted with budesonide and sand Ding Anehun treatment and the control group treated with additional ipratropium bromide on the basis of the treatment adopted by control group. The clinical efficacy, the disappearance of symptoms and the change of pulmo- nary function indexes ( including respiration, heart rate, SaO2, FEV1 and FEVl ) of two groups were observed before and after treatment. Results The total effective rate in the observation group was 95.24%, which was higher than 80.95% in the con- trol group ( P 〈 0.05 ). After treatment, the time of disappearance of wheezing, cough and dyspnea in observation group were shorter than those in control group (P 〈0. Ol ). After treatment, heart rate and respiratory rate in observation group were sig- nificantly higher than those in control group. The levels of SaO2 , FEV1 and FEV1% in observation group were significantly higher than those in control group ( P 〈 0.01 or P 〈 0.05 ). Conclusion Budesonide, ipratropium bromide and salbutamol combined with oxygen atomization in the treatment of asthmatic pneumonia in children have definite clinical efficacy, which is helpful to accelerate the children's recovery and improve the quality of their life. It is worthy of popularized in clinic.
