2种牵引方式在闭合复位PFNA内固定治疗股骨粗隆间及粗隆下骨折临床指标的比较

摘　　要：目的探讨闭合复位股骨近端防旋髓内钉(PFNA)内固定治疗股骨粗隆间合并粗隆下骨折术中应用双反牵引复位器与牵引床的临床效果及安全性差异。方法回顾性分析自2013-01—2016-01诊治的40例股骨粗隆间合并粗隆下骨折的临床资料,根据术中牵引方式的不同分为A、B 2组,A组采用双反牵引复位器,B组采用牵引床。比较2组手术时间、导针插入失败次数、术中透视次数、术中出血量、术后并发症发生率等指标。结果 2组术中出血量比较差异无统计学意义,A组在手术时间、术中导针插入失败次数及术中透视次数方面优于B组,差异有统计学意义(P<0.05),2组术后并发症发生率比较差异无统计学意义(P>0.05)。结论与牵引床相比,双反牵引复位器有助于闭合复位手术治疗股骨粗隆间合并粗隆下骨折的复位,有利于导针的插入,减少术中透视次数。Objective To investigate the efficacy and security of the two-way traction or traction table during closedreduction and operating treatment for trochanteric and subtroehanterie femoral fractures. Methods Forty patients withtrochanteric and subtrochanteric femoral fractures from January 2013 to January 2016 in our hospital were retrospectivelyanalyzed. According to different traction methods, objects were divided into two groups. Patients of group A were treated withtwo-way traction closed reduction treatment while patients of group B were treated with traditional traction table closedreduction treatment. The time of surgery, the number of guide pin insertion failure, the amount of bleeding, the number ofintraoperative fluoroscopes, the incidence of postoperative complications were compared. Results The amount of bleeding ofthe two groups had no significant differences. The time of surgery, the number of guide pin insertion failure, the number ofintraoperative fluoroscopes in A group were significantly lower than those in B group, but there was no significant difference inthe postoperative complications rate between two groups. Conclusion Two-way traction device is helpful for closed reductionand minimally invasive treatment of trochanteric and subtrochanteric femoral fractures, guide of pin insertion and decrease ofradiation exposure.
