摘 要:目的探讨缺氧诱导因子-1α(hypoxia inducible factor-1α,HIF-1α)和2型谷氨酰胺酶(glutaminase type2,GLS2)在结直肠癌病理诊断和预后评估中的临床意义。方法选取84例结直肠癌患者作为研究对象,均行手术切除,取结直肠癌组织和癌旁组织作为样本,分别采用免疫组化法检测样本中HIF-1α和GLS2表达水平。分析HIF-1α和GLS2表达水平及与结直肠癌病理特征和预后的关系。结果结直肠癌组织中HIF-1α和GLS2表达阳性率分别为73.81%(62/84)和79.76%(67/84),均显著高于癌旁组织(P<0.05)。结直肠癌组织中HIF-1α和GLS2表达呈显著相关(P<0.01)。结直肠癌肿瘤组织中HIF-1α表达阳性与年龄、TNM分期、淋巴结转移和分化程度有关,GLS2表达阳性与年龄、浸润深度、淋巴结转移和分化程度有关。HIF-1α和GLS2蛋白阳性表达患者术后平均无进展生存期和总生存期分别为(25.63±3.78)和(39.28±4.13)个月以及(24.66±2.89)和(38.37±4.28)个月,均显著长于阴性表达患者(P<0.05)。结论 HIF-1α和GLS2在结直肠癌组织中呈高表达,与淋巴结转移、预后不良存在相关性,可作为结直肠癌诊断和预后评估的新靶点加以深入研究。Objective To investigate the clinical significance of hypoxia inducible factor-1α(HIF-1α) and glutaminase type 2(GLS2) in assessing the pathological diagnosis and prognosis of colorectal cancer. MethodsThe HIF-1α and GLS2 expression of carcinoma and pericarcinous tissues of 84 patients with colorectal cancer were detected by immunohistochemistry. The relationship between the expression levels of HIF-1α,GLS2 and the pathological features of colorectal carcinoma were analyzed. The relationship between the levels of HIF-1α and GLS2 and prognosis was analyzed. Results The positive rates of HIF-1α and GLS2 expression in colorectal cancer tissues were 73.81%(62/84) and 79.76%(67/84),respectively, and were significantly higher than those in pericarcinous tissues(P〈0.05). There was a significant correlation between the expression of HIF-1α and GLS2 in colorectal cancer tissues(P〈0.01). The positive expression of HIF-1α in colorectal cancer tissues was related to age,TNM stage, lymph node metastasis and differentiation. The positive expression of GLS2 was related to age,depth of invasion, lymph node metastasis and differentiation. The positive expression of HIF-1α protein and GLS2 protein expression in patients with postoperative average progression free survival and overall survival were(25.63±3.78) months and(39.28±4.13) months,(24.66±2.89) months and(38.37±4.28) months, significantly longer than the negative patients(P〈0.05). Conclusion HIF-1α and GLS2 are highly expressed in colorectal cancer tissues, and have a correlation with lymph node metastasis and poor prognosis. They can be used as a new target for the diagnosis and prognosis evaluation of colorectal cancer.
