标准与微通道经皮肾镜取石术治疗肾结石效果分析

摘　　要：目的:探讨肾结石分别采用标准通道和微通道经皮肾镜取石术治疗的临床效果。方法:依照随机数表法将182例肾结石患者均分为对照组(91例)与研究组(91例),对照组采用标准通道经皮取石术治疗,研究组采用微通道经皮取石术治疗,观察两组术中出血量,手术时间,下床活动时间,住院时间,术后24h患者静脉Hb和Scr水平,以及结石清除率和术后并发症。结果:两组住院时间比较无统计学差异(P>0.05);对照组术中出血量、下床活动时间及Ser水平大于研究组,而手术时间、Hb水平低于研究组,各项比较均存在统计学差异(P<0.05)。对照组结石清除率为93.41%,研究组结石清除率为84.62%,两组比较有统计学差异(P>0.05);对照组并发症总发生率为29.67%,与研究组的26.37%比较未见统计学差异(P>0.05)。结论:标准通道和微通道PCNL对肾结石均有较好的治疗效果,两者有各自优势,标准通道PCNL的结石清除率更大,而微通道PCNL的治疗安全性更高,临床治疗肾结石需依据病情综合分析选择合适的手术方法。Objective:To investigate the clinical effect of renal calculi using standard access and microchannel percutaneous nerve lithotomy.Methods:According to the random number table method,182 patients with kidney stones were divided into the control group(91 cases)and the study group(91 cases).The control group was treated by the standard channel percutaneous lithotomy.The study group was treated by microchannel percutaneous lithotomy.The blood loss,operation time,ambulation time,hospital stay,venous Hb and Scr levels,and the rate of stone clearance and postoperative complications.Results:There was no significant difference in hospitalization time between the two groups(P>0.05).The intraoperative blood loss,ambulation time and Ser level in the control group were greater than those in the study group,while the operation time and Hb level were lower than those in the study group Learning difference(P<0.05).The stone clearance rate of the control group was 93.41%,the stone clearance rate of the study group was 84.62%,there was a significant difference between the two groups(P>0.05);the complication rate in the control group was 29.67%,compared with 26.37%in the study group See statistical differences(P>0.05).Conclusion:The standard channel and microchannel PCNL have a good therapeutic effect on kidney stones,both have their own advantages,the standard channel PCNL stone clearance rate is greater,and the treatment of microchannel PCNL more secure,clinical treatment of kidney stones to be based on Comprehensive analysis of the disease to choose the appropriate surgical methods.
