摘　　要：目的分析近4年住院患者疾病谱特征和费用,为合理配置医疗资源提供参考依据。方法收集2013年-2016年某三甲医院住院病案首页资料,根据ICD-10进行分类,并采用秩和比法和帕累托法对疾病构成进行综合分析。结果 2013年-2016年住院患者性别差异有统计学意义(χ~2=155.855,P<0.01)。主要病种谱为呼吸系统、循环系统、消化系统、眼和附器疾病、妊娠分娩、损伤中毒、肿瘤、泌尿系统和肌肉骨骼。人均住院费用由2013年11 237.72元增加到2016年13 688.57元。结论人均住院费用逐年增加,合理控制医疗费用增加破在眉睫。医院应结合自身住院疾病谱特点及规律,制定相关的卫生保障措施,更好地保障当地就医人群健康。Objective To analyze the characteristics of disease spectrum and cost in inpatients during recent four years, which is aimed to provide reference for allocating medical resources reasonably. Methods Collecting the medical record front pages of inpatients in a grade-A hospital in 2013-2016.The diseases identities of the included cases were counted according to ICD-10. Rank-sum ratio and Pareto analysis method were used to analyze the disease composition. Results There was statistical significance in gender of inpatients from 2013-2016（χ~2=155.855,P0.01）. The main diseases categories of this hospital were respiratory system diseases,circulatory system diseases,digestive system diseases,eye and attachment system,pregnancy childbirth and the puerperium,injuries and poisoning and certain other consequences of external cause, cancer, genitourinary system diseases,musculoskeletal system and connective tissue diseases. The per capital hospitalization cost increased from 11 237.72 yuan in 2013 to 13 688.57 yuan in 2016. Conclusions The average hospitalization cost increased year by year, the reasonable control of medical cost increase is around the corner. The hospital should combine the characteristics and rules of the disease spectrum in the hospital, and formulate relevant health care measures to better protect the health of the local residents.
