摘 要:目的分析经皮二氧化碳测定在高频辅助呼吸治疗新生儿呼吸窘迫综合征中的临床应用效果。方法选取2014年3月-2016年3月期间该院收治的30例新生儿呼吸窘迫综合征患儿,按照随机数字表法分为治疗组和对照组,每组各15例。对照组患儿应用抽血测定方法进行监测,观察组则应用经皮二氧化碳分压测定方法进行监测。比较两组患者治疗后不同时间段氧分压、二氧化碳分压、氧合指数、临床治疗效果以及治疗后相关并发症发生情况。结果治疗后,两组患儿的动脉血氧分压在治疗后显著上升,动脉血二氧化碳分压和氧合指数则明显下降。但在治疗2 h后,观察组患儿的动脉血氧分压、二氧化碳分压以及氧合指数情况与对照组患儿相比差异无统计学意义(P>0.05)。观察组患儿与对照组患儿总有效率一致,两组比较差异无统计学意义(P>0.05)。结论经皮二氧化碳测定辅助高频呼吸治疗新生儿呼吸窘迫综合征具有无创、连续等优点,在临床治疗中可以真实有效的反应患儿的二氧化碳分压数情况,减少对患儿的采血次数和并发症发生。Objective To analyze the clinical application effect of percutaneous carbon dioxide measurement for treatment of neonatal respiratory distress syndrome (NRDS) by high-frequency assisted respiration.Methods Thirty neonates with NRDS treated in Shenzhen Nanshan Hospital Affiliated to Guangdong Medical College from March 2013 to March 2016 were selected and divided into treatment group and control group according to random number table,15 neonates in each group.The neonates in control group were treated by blood drawing measuring method,and the neonates in observation group were treated by transcutaneous carbon dioxide partial pressure measuring method.The levels of oxygen partial pressure,partial pressure of carbon dioxide,oxygenation index at different time periods after treatment,clinical curative effects,and related complications in the two groups were compared.Results After treatment,the levels of oxygen partial pressure in the two groups increased significantly,while the levels of partial pressure of carbon dioxide and oxygenation index in the two groups decreased significantly.At two hours after treatment,there was no statistically significant difference in the levels of oxygen partial pressure,partial pressure of carbon dioxide,and oxygenation index between the two groups (P〉0.05).There was no statistically significant difference in the total effective rate between the two groups (P〉0.05).Condusion Percutaneous carbon dioxide measurement has the advantages of non-trauma and consecutiveness for treatment of NRDS by high-frequency assisted respiration,which can truly and effectively reflect partial pressure of carbon dioxide,reduce the number of blood collection and neonatal complications.
