双胎胎儿各孕周脾脏大小及脐血流变化与双胎围生儿不良预后的临床关系研究

摘　　要：目的探讨双胎胎儿各个孕周脾脏大小及脐血流变化与双胎围生儿不良预后的临床关系。方法选取2013年1-12月该院收治的双胎妊娠孕妇80例为研究对象,检测胎儿脐带血流变化情况以及各个孕周脾脏大小,分析同围生儿不良预后之间的关系。结果脐动脉血流收缩期末与舒张期末峰值速率比(S/D)值异常组的围生儿不良预后发生率明显高于正常组,差异有统计学意义(均P<0.05);脐动脉血流搏动指数(PI)值异常组的围生儿不良预后发生率明显高于正常组,差异有统计学意义(均P<0.05);各个孕周脾脏大小异常组的围生儿不良预后发生率明显高于正常组,差异有统计学意义(均P<0.05)。结论双胎胎儿各个孕周脾脏大小及其血流变化与双胎围生儿不良预后之间有密切的关系,应当引起医务人员的重视。Objective To explore the clinical relationships between spleen sizes and umbilical blood flow changes during different gestational weeks and perinatal adverse prognosis of twins.Methods Eighty women of twin pregnancy treated in Maternal and Child Health Care Hospital of Huadu District from January to December in 2013 were selected,umbilical blood flow changes cord and spleen sizes during different gestational weeks were detected,the relationships with perinatal adverse prognosis of twins were analyzed.Results The incidence rates of adverse prognosis in perinatal infants with abnormal S/D ratio were statistically significantly higher than those in perinatal infants with normal S/D ratio （P〈0.05）.The incidence rates of adverse prognosis in perinatal infants with abnormal pulsatility index （PI） of umbilical artery were statistically significantly higher than those in perinatal infants with normal PI of umbilical artery （P〈0.05）.The incidence rates of adverse prognosis in perinatal infants with abnormal spleen size were statistically significantly higher than those in perinatal infants with normal spleen size （P〈0.05）.Conclusion Spleen size and blood flow changes of twins during different gestational weeks are closely correlated with perinatal adverse prognosis of twins,the medical personnel should pay more attention to this phenomenon.
