2型糖尿病患者血清中脂蛋白相关磷脂酶A2检测的临床意义

摘　　要：目的通过检测2型糖尿病患者血清中脂蛋白相关磷脂酶A2(Lp-PLA2)水平,分析Lp-PLA2与2型糖尿病的关系,并进一步探讨Lp-PLA2在2型糖尿病并发症的发生、发展中的作用及临床意义。方法选择陕西省人民医院内分泌科2型糖尿病住院患者189例,将其分为3组,包括单纯2型糖尿病患者49例,2型糖尿病并发高脂血症患者56例,2型糖尿病并发动脉粥样硬化患者84例,同期选择正常对照组65例,分别检测Lp-PLA2,HbA1c质量水平,对各组间的结果进行比较。结果单纯2型糖尿病组(184.83±23.33ng/ml)、2型糖尿病并发高脂血症组(204.06±28.05ng/ml)、2型糖尿病并发动脉粥样硬化组(231.50±38.19ng/ml)Lp-PLA2水平均高于对照组(88.51±21.91ng/ml),四组间的差异具有统计学意义(F=194.78,P<0.01)。在单纯2型糖尿病组中对HbA1c与Lp-PLA2进行Pearson相关分析,结果显示2型糖尿病患者HbA1c与Lp-PLA2呈显著正相关(r=0.33,P<0.05)。结论 Lp-PLA2参与了2型糖尿病及其并发症的发生、发展过程,定期监测相关指标对预防、治疗和减少糖尿病患者并发症的发生有着非常重要的临床意义。Objective To analysis of the relationship between Lp-PLA2 and type 2 diabetes,and to further explore the Lp-PLA2 in complications of type 2 diabetes in the occurrence and development of the role and clinical significance through the detection of serum in patients with type 2 diabetes mellitus lipoprotein associated phospholipase A2（Lp-PLA2）level.Meth-od 189 patients with type 2 diabetes in the department of endocrinology of shaanxi people＇s hospital were selected and di-vided into 3 groups,including 49 patients with type 2 diabetes mellitus,type 2 diabetes mellitus with hyperlipidemia patients 56 cases,type 2 diabetes mellitus with atherosclerosis 84 cases.At the same time,65 cases of normal control group were se-lected.The levels of Lp-PLA2 and HbA1c were detected,and the results were compared among the groups.Results The levels of Lp-PLA2 in type 2 diabetes mellitus group（184.83±23.33 ng/ml）,type 2 diabetes mellitus complicated with hy-perlipidemia group（204.06 ± 28.05 ng/ml）,type 2 diabetes mellitus complicated with atherosclerosis group（231.50 ± 38.19 ng/ml）were higher than those in control group（88.51±21.91 ng/ml）,and the difference between the four groups was statistically significant（F=194.78,P〈0.01）.In the simple type 2 diabetes group.Pearson correlation analysis was performed between HbA1c and Lp-PLA2.The results showed that there was a significant positive correlation between HbA1c and Lp-PLA2 in patients with type 2 diabetes mellitus（r=0.33,P〈0.05）.Conclusion Lp-PLA2 is involved in the occurrence and development of type 2 diabetes mellitus and its complications.Regular monitoring of related indicators is of great clinical significance for prevention,treatment and reduction of complications in diabetic patients.
