经会阴三维超声对选择性剖宫产后盆底功能变化的评价作用

摘　　要：目的探讨经会阴三维超声在选择性剖宫产后盆底功能变化评价中的应用价值。方法选取选择性剖宫产初产妇92例根据产后是否存在盆底功能障碍(FPFD)性疾病23例为观察组,无FPFD性疾病产妇69例为对照组。产后6~8周行经会阴三维超声检查,观察两组产妇尿道转移角度、膀胱颈下移距离及不同状态下盆膈裂孔左右径、盆膈裂孔前后径、盆膈裂孔面积。结果观察组产妇尿道转移角度、膀胱颈下移距离以及静息、缩肛、Valsalva动作状态下盆膈裂孔左右径、盆膈裂孔前后径、盆膈裂孔面积同对照组比较,显著性均较大,差异有统计学意义(P<0.05)。结论经会阴三维超声可准确反映选择性剖宫产后盆底功能变化,可为产后FPFD性疾病诊断提供有效参考,值得推广。Objective To evaluate the application value of transperineal three - dimensional ultrasound in evaluating the changes of pelvic floor function after selective cesarean section. Methods Ninety - two cases of primiparas for selective cesarean section were selected as the research objects and were divided into the observation group with 23 cases of primiparas with FPFD disease and the control group with 69 cases of primiparas without FPFD disease according to whether they suffered from pelvic floor dysfunction （ FPFD ） after delivery or not. The primiparas in two groups were examined by transperineal three - dimensional ultrasound at 6 - 8 weeks after deliver- y. The angle of urethral metastasis, the distance of the downward movement of the bladder neck and the left and right diameter of the hiatus of pelvis under different conditions, anterior and posterior diameter of pelvic diaphragm hiatus and the area of the pelvic diaphragm hiatus in the primiparas of two groups were observed. Results The angle of urethral metastasis, the distance of the downward movement of the bladder neck, tranquillization, anal contraction, left and right diameter of the pelvic diaphragm hiatus in the Valsalva action state, anterior and posteri-or diameter of pelvic diaphragm hiatus and the area of the pelvic diaphragm hiatus in the primiparas in the observa-tion group were significantly higher than those in the control group, and the difference was statistically significant （P 〈0. 05） . Conclusion Transperineal three - dimensional ultrasound can accurately reflect the change of pel-vic floor function after selective cesarean section and can provide an effective reference for the diagnosis of postpar-tum FPFD disease, which is worthy of promotion.
