改良3M敷贴在儿童颈外中心静脉的临床应用

摘　　要：目的观察3M加压固定带在中心静脉导管(CVC)固定中的应用效果。方法选取2015年7月~12月住院术后留取颈外双腔CVC导管的患者60例为对照组,采用常规3M透明贴膜进行导管固定;2016年1月~6月住院患儿留取颈外双腔CVC导管的患儿60例为观察组,采用改良后中心无菌敷料加3M敷贴的覆盖方法进行CVC导管的固定。比较两组患者导管是否脱出率、敷贴松动、敷贴更换时间、敷贴使用的天数、CVC留置天数、感染率。结果观察组导管托出率、敷贴松动率、更换敷贴次数、敷贴使用时间、CVC留置时间明显优于对照组,数据差异明显,具有统计学意义,(P<0.05)观察组无1例发生CVC感染,对照组发生1例CVC感染(1.67%),两组患儿CVC感染率之间差异不具有统计学意义(P>0.05)。结论试验组固定CVC导管的方法优于常规组,固定效果好,具有临床意义。Objective To observe the effect of 3M compression band in the fixation of central venous catheter （CVC）. Methods 60 patients from July 2015 to December with dual lumen CVC catheter were chosen as the control group？ and con-ventional 3M transparent film was used for catheter fixation. 60 cases of children from January to April 2016 with dual lumenCVC catheter were taken as observation group, and the CVC catheter was fixed with modified central sterile dressing plus 3Mdressing. The catheter release rate, patch loosening, patch replacement time？ the number of days applied？ the number ofCVC days and the rate of infection were compared between the two groups. Results The observation group catheter out rate, sticking loose rate, dressing change times, dressing time, indwelling time of CVC in the observation group was significantlybetter than the control group（P〈0 . 05）. 1 cases of CVC infection occurred in the control group （1. 67%） , and no case oc-curred in the observation group. It was no statistically significant in the infection rate of CVC between the two groups of chil-dren. Conclusion the method of CVC catheter fixation in experimental group is superior to conventional group 〉 and the fixa-tion effect is good and has clinical significance.
