毛细支气管炎后反复喘息患儿激素疗程防治效果及对患儿身体发育的影响

摘　　要：目的研究毛细支气管炎后反复喘息患儿激素疗程防治效果及对患儿身体发育的影响。方法选取我院2012年3月~2015年9月144例毛细支气管炎患儿为研究对象,将患者随机分为观察组和对照组,每组72例,观察组在毛细支气管炎治愈后采用糖皮质激素治疗,对照组在毛细支气管炎治愈后不进行干预,比较两组的喘息发作情况、肺功能参数以及身体发育情况。结果观察组的喘息发作率、喘息持续时间分别为40.3%、(3.13±2.09)d显著低于对照组的62.5%、(6.27±3.18)d(P<0.05)。观察组的无症状天数、潮气量(VT)分别(85.63±3.31)d、(7.52±1.29)%为显著高于对照组的(79.05±7.52)d、(6.74±1.44)%(P<0.05)。观察组随访1年后的身高、体重、身体质量指数(BMI)与对照组比较,无显著差异(P>0.05)。结论毛细支气管炎后反复喘息患儿激素疗程,可以减轻肺功能损伤,预防反复喘息的发生,不会对患儿生长发育造成显著影响,值得临床应用和推广。Objective To investigate the effect of hormone therapy on repeated wheeze outbreak in bronchiolitis and onchildren’s body development. Methods A total of 144 children with bronchiolitis from March 2012 to September 2015 wererandomize into the observation group and control group, 72 cases in each group. The control group was treated with glucocor-ticoid after bronchiolitiscured, while the control group was not treated. The wheezing, pulmonary function and body develop-ment were compared between the two groups. Results The rate of wheezing and wheezing duration in the observation groupwere significantly lower than those in the control group （40. 3% vs 62. 5% ,（ 3. 1 3 ± 2 . 09）d vs （6. 2 7 ± 3 . 1 8 ）d ,both （ P 〈 0. 05）. The number of asymptomatic days and tidal volume in the observation group were significantly higher than those in thecontrol group （85. 63 ± 3. 31）d vs （79. 05 ±7. 52 ）d,（ 7. 52± 1. 2 9 ）% vs （6. 74 ± 1. 4 4 ）% ,both P〈0 . 05）. There was nosignificant difference in height, body weight and body mass index （BMI） between the observation group and the control groupafter one year followed up （P〉0. 05）. Conclusion Hormone therapy can reduce lung injury and prevent the occurrence of re-peated wheezing of children with repeated wheeze outbreak in bronchiolitis. Hormone therapy has no influence on the growthand development of children, worthy of clinical application.
