糖尿病视网膜病变超声乳化玻璃体切除硅油填充术后高眼压的危险因素

摘　　要：目的 分析增生性糖尿病视网膜病变（PDR）患者行超声乳化联合玻璃体切除硅油填充术后高眼压的危险因素.方法 回顾性分析2013年1月至2016年3月32例（32只眼）PDR行超声乳化联合玻璃体切除硅油填充术的临床资料,分析术后3个月高眼压的相关危险因素.结果 术后随访3个月高眼压共10例（10只眼）,发生率为31.25%;术后1周、术后1个月眼压均较术前高（P〈0.05）.术后视力较术前均有改善（P=0.048,0.000）,术后3个月高眼压者视力低于正常眼压者（P=0.040）.糖尿病病程〉10年为高眼压的独立危险因素（O R=0.268,P=0.040）.结论 PDR患者行超声乳化联合玻璃体切除硅油填充术后视力改善明显,但高眼压影响视力恢复,术后1周、1个月出现高眼压的概率较大.糖尿病病程〉10年为高眼压的危险因素.Objective To analyse risk factors of ocular hypertension after vitrectomy combined with phacoemulsification and silicone oil tamponade for proliferative diabetic retinopathy （PDR）.Methods Clinical datas of 32 eyes of 32 patients with PDR from Jan .2013 to Mar.2016 were analyzed retrospectively .All cases had received vitrectomy combined with phacoemulsification and silicone oil tamponade .The risk factors of ocular hypertension 3 months after surgery were analyzed .Results The follow-up time was 3 months after surgery, ocular hypertension occurred in 10 eyes of 10 cases （31.25%）.The IOP at 1 week and 1 month after surgery were higher than those before surgery （P〈0.05）.The visual acuity after operation were higher than that before surgery （P=0.048, 0.000）.The visual acuity of ocular hypertension cases at 3 months after surgery were lower than those of normal IOP cases （P=0.040）.The diabetic duration 〉10 years was one independent risk factor of ocular hypertension （OR=0.268, P=0.040）.Conclusion The visual acuity can be improved post-operation after vitrectomy combined with phacoemulsification and silicone oil tamponade for PDR , while ocular hypertension is still a common complication leading severe vision impairment especially at 1 week and 1 month post-operatively .Diabetic duration over 10 years is risk factor inducing IOP elevation .
