摘 要:目的:探讨神经节苷脂对结肠癌患者含奥沙利铂化疗方案致周围神经毒性的疗效。方法:选取结肠癌患者60例,以随机数字表法分为观察组和对照组,每组30例。两组患者均给予奥沙利铂+亚叶酸钙+氟尿嘧啶方案或奥沙利铂+卡培他滨方案化疗。对照组患者于每个化疗周期第1日给予注射用还原型谷胱甘肽,1次0.3 g,1日1次,静脉滴注,连续给药14 d;观察组患者于每个化疗周期第1日给予单唾液酸四己糖神经节苷脂钠注射液,1次20 mg,1日1次,静脉滴注,连续给药14 d。两组患者均以21 d为1个化疗周期。观察两组患者周围神经毒性及其他不良反应发生情况,比较两组患者化疗疗效的差异。结果:化疗过程中,观察组患者周围神经毒性发生率为70.00%(21/30),明显低于对照组的83.33%(25/30),差异有统计学意义(P<0.05);两组患者均未发生Ⅳ级周围神经毒性,观察组患者Ⅱ、Ⅲ级周围神经毒性发生率明显低于对照组,差异均有统计学意义(P<0.05);两组患者其他不良反应发生率的差异无统计学意义(P>0.05);两组患者客观缓解率、疾病控制率的差异均无统计学意义(P>0.05)。结论:神经节苷脂可降低含奥沙利铂化疗方案致结肠癌患者周围神经毒性的发生率,并可减轻周围神经毒性反应程度。OBJECTIVE: To probe into the efficacy of gangliosides in application of chemotherapy regimen of oxaliplatin in treatment of peripheral neurotoxicity among patients with colon cancer. METHODS: 60 patients with colon cancer were selected and divided into observation group and control group via random number table,with30 cases in each. Both groups were treated with oxaliplatin + calcium folinate + 5-fluorouracil or oxaliplatin +capecitabine. The control group was given reduced glutathione injection for intravenous drip at the first day of each chemotherapy course for 0. 3 g once a day,with continuous administration of 14 days; the observation group was given monosialotetrahexosyl ganglioside sodium injection for intravenous drip at the first day of each chemotherapy course for20 mg once a day,with continuous administration of 14 days. 21 days were set as a chemotherapy course for each group. Peripheral neurotoxicity and other adverse drug reactions of two groups were observed,and the difference of efficacy between two groups were compared. RESULTS: During chemotherapy, the incidences of peripheral neurotoxicity of observation group( 70. 00%,21/30) was significantly lower than that of the control group( 83. 33%,25/30),with statistically significant difference( P〈0. 05); no Ⅳ level of peripheral neurotoxicity was found in both groups,the incidence of Ⅱ level and Ⅲ level of peripheral neurotoxicity of observation group was significantly lower than that of the control group,with statistically significant difference( P〈0. 05); there was no statistical significance in difference of other incidences of adverse drug reactions between two groups( P〈0. 05); there was no statistical significance in differences of the objective remission rates and disease control rates between two groups( P〈0. 05).CONCLUSIONS: Gangliosides can reduce the incidence of peripheral neurotoxicity in chemotherapy regimen ofoxaliplatin among patients with colon cancer,and relieve the peripheral neuro
