茵栀黄注射液辅助治疗新生儿高胆红素血症的系统评价

摘　　要：目的:系统评价茵栀黄注射液辅助治疗新生儿高胆红素血症的有效性,为茵栀黄注射液在儿科临床的应用提供循证参考。方法:计算机检索中文科技期刊数据库、中国期刊全文数据库、万方数据库和Pub Med等数据库,收集在包含蓝光照射等常规治疗(对照组)基础上联合应用茵栀黄注射液(观察组)治疗新生儿高胆红素血症的随机对照试验,对符合纳入标准的临床研究进行资料提取和质量评价后,进行文献荟萃(Meta)分析。结果:共纳入13篇文献,涉及1 864例患儿。Meta分析结果显示,观察组患儿的总有效率(OR=5.70,95%CI=4.12~7.87,P<0.000 01)、胆红素降至正常水平所需时间(MD=-2.25,95%CI=-2.98^-1.53,P<0.000 01)和治疗后胆红素水平(MD=-34.15,95%CI=-47.10^-21.21,P<0.000 01)均明显优于对照组,差异均有统计学意义。结论:茵栀黄注射液联合蓝光照射等常规治疗对新生儿高胆红素血症的效果较好。OBJECTIVE： To systematically review the effectiveness of Yinzhihuang injection in adjuvant treatment of neonatal hyperbilirubinemia,so as to provide evidence-based reference for its clinical application in pediatrics. METHODS：Randomized controlled trials of conventional therapy（ control group） including blue light irradiation combined with Yinzhihuang injection（ observation group） in treatment of neonatal hyperbilirubinemia were retrieved from Chinese Science and Technology Periodical Full-text Database,Chinese Journal Full-text Database,Wanfang Database,Pub Med,etc.Clinical studies matched the inclusion criteria were involved to conduct data extraction,quality evaluation and Meta analysis. RESULTS： A total of 13 studies were involved,including 1864 child patients. Results of Meta analysis showed the total effective rate（ OR = 5. 70,95% CI = 4. 12-7. 87,P〈0. 000 01）,required time for bilirubin level reduced to normal（ MD =-2. 25,95%CI =-2. 98--1. 53,P〈0. 000 01） and bilirubin level after treatment（ MD =-34. 15,95% CI =-47. 10--21. 21,P〈0. 000 01） of observation group were significantly better than those of the control group,with statistically significant differences. CONCLUSIONS： The efficacy of conventional therapy including blue light irradiation combined with Yinzhihuang injection in treatment of neonatal hyperbilirubinemia is remarkable.
