摘 要:目的探讨伤椎置钉联合经椎弓根椎体内植骨治疗胸腰椎骨折的临床疗效。方法回顾性分析48例采用伤椎置钉联合经椎弓根椎体内植骨术治疗的胸腰椎骨折患者的临床资料,观察患者术前、术后1周、术后12个月X线片,比较手术前后Cobb角、伤椎相对高度;观察有无内固定失败(包括内固定折断、折弯,椎弓根钉拔出等)。结果所有患者均得到随访,无内固定松动、断裂等发生,术后12个月、术后1周患者Cobb角(3.42±3.73)、(2.56±1.17)°及伤椎相对高度(95.79±2.81)%、(96.54±3.43)%均优于术前(19.56±3.94)°、(55.43±14.58)%,差异具有统计学意义(P<0.05);术后12个月患者Cobb角及伤椎相对高度与术后1周比较,差异无统计学意义(P>0.05)。结论伤椎置钉联合经椎弓根椎体内植骨治疗可有效恢复胸腰椎骨折患者的伤椎椎体高度,重建脊柱的稳定性,减少并发症,明显促进患者的术后康复。Objective To discuss the clinical efficacy of pedicular screw and transpedicular bone grafting in the treatment of thoracolumbar fractures. Methods The clinical data of 48 thoracolumbar fractures patients treated with pedicular screw and transpedicular bone grafting was retrospectively analyzed. The X-ray film before operation and 1 weeks and 12 months after operation were observed, and the Cobb angle and relative height of the injured vertebra before and after operation were compared. Observation was made on whether there was internal fixation failures(including internal fixation, bending, pedicle screw extraction, etc.). Results All patients were followed up without any loosening or breakage of the internal fixation. Patients had Cobb angel in postoperative 12 months and 1 weeks as(3.42±3.73) and(2.56±1.17)°, and relative height of injured vertebra as(95.79±2.81)% and(96.54±3.43)%, which were all better than(19.56±3.94)°and(55.43±14.58)% before treatment. Their difference was statistically significant(P〈0.05). patients had no statistically significant difference in Cobb angel and relative height of injured vertebra in postoperative 12 months and 1 weeks(P〉0.05).Conclusion Combined therapy of pedicular screw and transpedicular bone grafting can effectively restore the vertebral height, rebuild the stability of the spine, reduce the complications, and promote the postoperative rehabilitation of patients with thoracolumbar fractures.
