经输尿管镜钬激光碎石术与气压弹道碎石术治疗输尿管中下段结石的临床对比

摘　　要：目的对经输尿管镜钬激光碎石术与气压弹道碎石术在输尿管中下段结石治疗中的疗效进行探讨。方法 88例输尿管中下段结石患者,依据治疗方法的不同分为对照组(经输尿管镜气压弹道碎石术)与观察组(经输尿管镜钬激光碎石术),每组44例。观察两组患者的临床疗效。结果观察组患者的手术时间为(46.38±7.30)min,短于对照组的(56.35±9.12)min,差异有统计学意义(P<0.01)。观察组患者住院时间略短于对照组,但差异无统计学意义(P>0.05)。对照组患者的碎石成功率为86.36%,低于观察组的97.73%,差异有统计学意义(P<0.05)。观察组患者的并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。结论经输尿管镜钬激光碎石术治疗输尿管中下段结石成功率高,手术时间短,减少了肾盂内持续高压时间,降低了尿源性脓毒血症发生,更有利于患者的治疗与康复。Objective To investigate the efficacy of ureteroscopic holmium laser lithotripsy and pneumatic lithotripsy in the treatment of middle and lower uretm＇al calculi. Methods A total of 88 patients with middle and lower ureteral calculi were divided by different treatment methods into control group （ureteroscopic pneumatic lithotripsy） and observation group （ureteroscopic holmium laser lithotripsy）, with 44 cases in each group. The clinical efficacy in two groups was observed. Results The observation group had shorter operation time as （46.38 ＋ 7.30） min than （56.35 ＋ 9.12） rain in the control group, and the difference was statistically significant （P〈0.01）. The observation group bad a little shorter hospitalization time than the control group, but the difference was not statistically significant （/~〉0.05）. The control group had lower successful rate of lithotripsy as 86.36% than 97.73% in the observation group, and the difference was statistically significant （P〈0.05）. The observation group had obviously lower incidence of complications than the control group, and the difference was statistically significant （P〈0.05）. Conclusion Ureteroscopic holmium laser lithotripsy shows high success rate, and can shorten operation time, reduce continuous high pressure time in the pelvis, and lower the occurrence of urinary sepsis. It is more conducive to the treatment and rehabilitation of the patients.
