摘 要:目的通过定量测定乙型肝炎患者血浆游离线粒体DNA拷贝数水平,探讨线粒体DNA拷贝数变化与临床常用检测指标的关系及其在疾病评估上的应用价值,为肝损伤患者所释放的线粒体DNA导致的全身无菌性炎症反应预测、诊断及治疗提供依据。方法采集50名健康体检者标本(对照组),50例乙型肝炎患者(乙肝组),应用荧光定量聚合酶链反应(PCR)法测定其循环游离线粒体DNA拷贝数,同时测量对照组与乙肝组谷氨酸脱氢酶、白细胞介素-6、白细胞水平。结果对照组血浆游离线粒体DNA拷贝数为4.0×10~4(7.0×10~4)拷贝/ml;乙肝组血浆游离线粒体DNA拷贝数为4.2×10~5(1.1×10~6)拷贝/ml,乙肝组浆游离线粒体DNA拷贝数升高(P<0.05)。乙肝组与对照组相比血清谷氨酸脱氢酶、白细胞介素-6、白细胞升高(P<0.05)。结论乙肝患者血浆游离线粒体DNA水平增加,血浆游离线粒体DNA介导炎性因子白细胞介素-6释放,可作线粒体功能障碍的生物标志物。Objective To quantitate the level of plasma circulating cell-free mitochondrial DNA(mt DNA) copy number in patients with hepatitis B, and to explore the relationship between altered mt DNA copy number and clinical investigation findings and its value in disease evaluation, so as to provide evidence for prediction, diagnosis and man-agement of systemic aseptic inflammation induced by mt DNA. Methods Fifty healthy subjects were recruited as control group and 50 hepatitis B patients as HBV group. The circulating cell-free mt DNA copy number was measured by real-time quantitative PCR. Meanwhile, the levels of glutamate dehydrogenase, Interleukin 6, leukocytes were ex-amined. Results The plasma circulating cell-free mt DNA copy number was 4.0×10^4(7.0×10^4) copies/ml in the control group, compared with 4.2 ×10^5 copies(1.1 ×10^6) copies/ml in the HBV group, suggesting that plasma circulating cell-free mt DNA copy number was increased in the hepatitis B patients(P〈0.05). Compared with control group, levels of serum glutamate dehydrogenase, interleukin 6(IL-6) and leukocytes were increased in the HBV group(P〈0.05).Conclusion The level of plasma circulating cell-free mt DNA in patients with hepatitis B is increased. Plasma circulating cell-free mt DNA may be involved in the secretion of IL-6, and can be used as a biomarker of mitochondrial dysfunction.
