摘  要:探讨慢性中耳炎细菌培养及细菌生物膜形成的关系分析。选取2015年2月~2017年2月在我院治疗的慢性中耳炎患者143例,其中慢性化脓性中耳炎(活动期)79例,中耳胆脂瘤64例,对患者中耳分泌物进行细菌培养鉴定,同时进行扫描电子显微镜检查。慢性化脓性中耳炎和中耳胆脂瘤患者细菌生物膜阳性率分别为82.28%和71.88%,差异比较无统计学意义(p>0.05);143例标本中共有108例检出病原菌,检出率为75.52%;慢性化脓性中耳炎和中耳胆脂瘤细菌培养阳性率分别为72.15%和79.69%,差异比较无统计学意义(p>0.05);慢性化脓性中耳炎和中耳胆脂瘤细菌类型分布比较无统计学意义(p>0.05);细菌培养结果与细菌生物膜结果无相关性(p>0.05);以细菌培养结果预测细菌生物膜的灵敏度、特异度、阳性预测值和阴性预测值分别为76.58%、28.13%、78.70%和25.71%;不同细菌生物膜阳性率比较差异无统计学意义(p>0.05)。慢性中耳炎细菌生物膜形成率较高,与细菌培养结果无相关性。To investigate the bacterial culture and bacterial biofilm formation in chronic otitis media, 143 cases of chronic otitis media treated in our hospital from February 2015 to February 2017 were selected, among them, 79 cases were chronic suppurative otitis media (active stage), and 64 cases with cholesteatoma in the middle ear, bacterial culture and identification of middle ear secretions were performed and scanning electron microscopy was performed. The positive rates of bacterial biofilms in chronic suppurative otitis media and middle ear cholesteatoma were 82.28% and 71.88%, respectively, and the difference was not statistically significant (p 〉0.05); pathogens were detected in 108 of the 143 specimens, the detection rate was 75.52%; chronic suppurative otitis media and middle ear cholesteatoma positive rate of bacterial culture were 72.15% and 79.69%, the difference was not statistically significant (p 〉0.05); there were no statistically significant differences in bacterial distribution between chronic suppurative otitis media and middle ear cholesteatoma (p 〉0.05); the result of bacterial culture was not related to bacterial biofilm (p 〉0.05); the sensitivity, specificity, positive predictive value and negative predictive value of bacterial biofilm was determined by bacterial culture were 76.58%, 28.13%, 78.70% and 25.71%, respectively; There was no significant difference in the positive rates of different bacterial biofilms (p〉0.05). The rate of bacterial biofilm formation is high in chronic otitis media, has no correlation with bacterial culture.
