摘　　要：目的探讨血清胱抑素-C(cystatin,Cys-C)在诊断2型糖尿病患者早期肾损伤中的意义。方法选取本院2015年1月~2017年3月间收治的2型糖尿病患者90例,检测患者的空腹血清Cys-C、血肌酐(SCr)及尿白蛋白/肌酐比值(albumin creatinine rate,ACR)水平。根据ACR水平分为早期糖尿病肾病组及单纯糖尿病组,比较两组患者的Cys-C及SCr水平,并进行统计学分析。结果两组患者血清肌酐水平无明显差异,早期糖尿病肾病组其血清Cys-C水平显著高于单纯糖尿病组,差异具有统计学意义(P<0.05)。Pearson相关分析显示血清Cys-C表达水平与ACR水平呈正相关(r=0.678,P<0.05)。结论血清Cys-C测定可有效判断糖尿病患者早期肾脏损伤,可用于早期肾损伤的诊断。Objective To investigate the significance of serum cystatin-C（Cys-C） in the diagnosis of early renal injury in type 2 diabetic patients. Methods 90 patients with type 2 diabetes mellitus admitted from January 2015 to March 2017 in our hospital were enrolled in this study. The fasting serum Cys-C, serum creatinine（SCr） and urinary albumin/ creatinine（ACR） levels in patients were measured. According to the levels of ACR, the patients were divided into early diabetic nephropathy group and simple diabetic group. The levels of Cys-C and SCr in the two groups were compared and statistically analyzed. Results There was no significant difference in serum creatinine between the two groups. The serum Cys-C level in diabetic nephropathy group was significantly higher than that in simple diabetic group, and the difference was significant（P〈0.05）. Pearson correlation analysis showed that there was a positive correlation between serum Cys-C level and ACR level （r=0.678, P〈0.05）. Conclusion Serum Cys-C detection can effectively determine the early diabetic kidney injury in diabetic patients and can be used for the diagnosis of early renal injury.
