补中益气汤熏洗对痉挛型脑瘫膝反张中长期疗效的观察

摘　　要：目的观察补中益气汤熏洗对痉挛型脑瘫膝反张的中长期疗效。方法用补中益气汤熏洗63例痉挛型脑瘫膝反张儿童,按时间不同分成3个月组(A组)、6个月组(B组)、12个月组(C组),每组21例。各组对应治疗时间结束后停止熏洗,但均观察并记录评估指标至12个月。以改良Ashworth量表(MAS),膝关节最大伸直度和粗大运动功能评定量表(GMFM)中D、E能区分值为疗效观察的指标。结果 C组的有效率95.24%,显著优于A组的71.43%和B组的85.71%,C组的MAS、膝关节最大伸直度评定分值较治疗前明显降低,GMFM的D、E能区评定分值较治疗前明显提高。C组与A组和B组比较,差异有统计学意义(P<0.01)。结论长期应用补中益气汤熏洗能改善痉挛型脑瘫膝反张的情况,可以较好地维持中短期的治疗效果,为临床用药提供参考依据。Objective To observe the medium and long-term curative effect of Buzhong Yiqi decoction on knee recurvatum of spastic cerebral palsy. Methods 63 children with knee recurvatum of spastic cerebral palsy were fumigated with Buzhong Yiqi decoction. The children were divided into 3-month group （group A）, 6-month group （group B）, 12- month group （group C） according to the time, with 21 cases in each group. Each group stopped fumigation after the end of corresponding treatment time, while the evaluation indicators were observed and recorded until 12 months. The modified Ashworth scale（MAS）, maximal straightness of knee joint and D, E energy zones assessment score of gross motor function assessment scale（GMFM） were chosen as the indexes for the efficacy of the observation. Results The effective rate of group C was 95.24%, which was significantly better than that of 71.43% in group A and 85.71% in group B. The assessment scores of MAS and maximum straightness of knee joint in group C after treatment were significantly lower than those before treatment, while the D, E energy zone assessment score of GMFM was significantly higher than that before treatment. There was significant difference between group C and group A, group B（P〈0.01）. Conclusion The long-term application of Buzhong Yiqi decoction fumigation can improve the knee recurvatum situation of spastic cerebral palsy, which can better maintain the short and medium-term treatment effect and provide a reference for clinical drug use.
