摘 要:目的研究中国长寿地区中、老年人肾功能指标尿素氮(blood urea nitrogen,BUN)、肌酐(creatinine,CRE)变化及其与高血压、糖尿病的关系。方法以中国8个长寿之乡参加2014年长寿县市"老年人口健康状况的生物医学研究"的居民作为目标对象,收集8个长寿地区2401名60岁以上受访对象社会人口学信息,对其按统一标准体检并采集空腹静脉血检测BUN和CRE。比较不同年龄不同性别组间的肾功能差异,以及高血压、糖尿病不同患病情况调查对象间BUN和CRE的差异。结果 8个长寿地区老年人BUN和CRE均随年龄增加而显著升高(P<0.001),男女性之间差异无统计学意义(P>0.05);BUN异常人群的年龄和糖尿病患病率高于正常人群,差异具有统计学意义;CRE异常人群的年龄和糖尿病及高血压的患病率高于正常人群,差异具有统计学意义(P<0.001)。结论 8个长寿地区老年人肾功能随年龄增加而降低;糖尿病是肾功能下降的重要危险因素,高血压患者CRE升高有统计学意义,BUN升高没有统计学意义。Objective To analyze the changes of the renal function in the elder people in longevity areas and the relationship between the renal function and hypertension and diabetes. Methods Information of 2401 people aged over 60 years old in 8 longevity areas was collected, including socio-demographic. Physical examination was conducted according to unified standards and fasting blood samples were collected for BUN and CRE. The differences of renal function between different age groups and different genders were compared, as well as the differences of renal function among people with hypertension or diabetes. Results In the 8 longevity areas, BUN and CRE of the elderly increased significantly with age, the difference between male and female at the same age group showed no significant difference. CRE in hypertension and diabetes groups was higher than that of the group without hypertensive and diabetes, the difference showed significant difference. Conclusion The results indicated that the renal function of the elderly in the 8 longevity areas significantly decreased with the increase of age, and hypertension and diabetes were the important risk factors for the decline of renal function.
