正常甲状腺功能病态综合征FT_3、FT_4和TSH与老年性疾病相关性研究

摘　　要：目的通过测定血清游离FT_3、FT_4和TSH含量,探讨不同疾病组老年患者甲状腺功能状况。方法用化学发光法测定我院226例老年患者血清FT_3、FT_4和TSH水平,并进行统计分析。结果 FT_3、FT_4和TSH测定结果分别为2.19pg/m L(中位数,P2.5~P97.5:1.00~3.12pg/m L)、1.01 ng/d L(中位数,P2.5~P97.5:0.67~1.44ng/d L)和1.44μIU/m L(中位数,P2.5~P97.5:0.28~4.92μIU/m L),FT_3和FT_4降低发生率分别为31.86%和3.54%,TSH增高发生率为3.10%。不同性别老年患者FT_3、TSH差异具有统计学意义,而FT_4差异无统计学意义。FT_3、FT_4和TSH异常发生率在两性之间差异均无统计学意义。FT_3和FT_4水平在不同疾病组之间差异显著,TSH在不同疾病组之间差异无统计学意义。FT_3和TSH异常发生率在不同疾病组之间差异显著,FT_4异常发生率在不同疾病组之间差异无统计学意义。结论在老年性疾病患者中正常甲状腺功能病态综合征甲状腺功能异常发生主要为FT_3降低。Objective Evaluate the status of thyroid function of elderly patients with different disease by investigating the levels of serum free FT3, FT4 and TSH. Methods Serum FT3, FT4 and TSH levels were measured by chemiluminescence and analyzed in 226 elderly patients in our hospital. Results The results of FT3, FT4 and TSH were 2.19 pg/mL（Median,2.5th-97.5th：1.00-3.12 pg/mL）、1.01 ng/dL（Median,2.5th-97.5th：0.67-1.44ng/dL）和1.44 μIU/mL（Median,2.5th-97.5th：0.28-4.92 μIU/mL）, respectively. The incidence of FT3 and FT4 was 31.86% and 3.54% , The incidence of TSH increased by 3.10%. There were significant differences in FT3 and TSH between different sex and elderly patients, but there was no significant difference between FT3 and FT4. There was no significant difference in the incidence of FT3, FT4 and TSH between the sexes. The levels of FT3 and FT4 between different disease groups showed significant differences. While TSH showed no differences among different disease groups. Abnormal rate of FT3 and TSH was significantly different among different disease groups. And bnormal rate of FT4 between different disease groups showed no significant difference. Conclusion The occurrence of thyroid dysfunction in elderly patients with Euthyroid sick syndrome is mainly reduced by FT3.
