摘  要:目的 探讨精确计算支气管斜位体层摄影新技术及其临床应用价值。方法 选择2 0例胸部标准CT片 ,取其左、右支气管分叉层面作为标准图像 ,再以患者实际胸廓的大小 ,将图像放大于坐标纸上。根据各种前、后支气管斜位体层公式中的角度绘制出对应的各条径线 ,进行测量分析。获得新的体层差值公式及取层方法 ,并据此设计出智能体层计算仪。应用新的体层取层方法 ,随机对 4 86例患者 896张支气管斜位体层摄影片进行效果分析 ,选择以传统取层方法摄影的 5 6 7例患者2 2 0 8张支气管体层片为对照组。结果 改良前 5 6 7例 ,使用胶片 2 2 0 8张 ,优良片 1174张 ,优良片率5 3 17%。改进后 4 86例 ,使用胶片 896张 ,优良片 881张 ,优良片率 98 33%。欲断支气管在 1~ 2张片内完成。两组相比差异具有非常显著性意义 (U =2 4 10 ,P <0 0 1)。结论 改进后的支气管斜位取层方法 ,具有操作简单、计算准确、速度快等优点 ,2~ 3级支气管显示清晰 。Objective To study accurate calculating technology for bronchi oblique tomography and its clinical significance. Methods According to 20 patients′ chest CT films (at the level of carina tracheae) and patients′ actual chest size, compared the measured oblique angle, longitude and other parameters with those calculated with old formula, a new formula of bronchi oblique tomography was deduced and a tomography computer was designed. Using the old method, 2 208 bronchi tomography radiographs in randomly selected 567 patients were chosen as control group. After improvement, 896 radiographs in 486 patients were compared with that in the control group. Results All radiographs were classified into four degrees as Grade A, B, C, and D. 881 of 896 radiographs were Grade A and B (98.33%) using the new method, while 1 174 of 2 208 were Grade A and B (53 17%) in the control group, and the difference in excellent rate between the two groups had statistically significance ( U =24.10, P <0.01). Conclusion The improved method of bronchi oblique tomography is accurate, simple, and rapid, with grade 2-3 bronchibeing displayed clearly, and it could increase the quality control in clinical practice.
