左炔诺孕酮宫内缓释系统与孕三烯酮在子宫腺肌病治疗中的疗效对比

摘　　要：目的 比较左炔诺孕酮宫内缓释系统（LNG-IUS）与孕三烯酮在子宫腺肌病治疗中的疗效.方法 选取2014月1月—2016年1月茂名市人民医院收治的子宫腺肌病患者120例,随机分为观察组及对照组各60例.对照组患者给予孕三烯酮治疗,观察组患者给予LNG-IUS治疗.比较两组患者治疗前及治疗3个月、12个月后痛经程度、子宫大小、月经量及不良反应情况.结果 两组患者治疗3个月后痛经程度、子宫内膜厚度及大小、月经量明显改善,与治疗前相比差异有统计学意义（P〈0.05）.治疗3个月及12个月后观察组患者痛经程度明显低于对照组,差异有统计学意义（P〈0.05）;观察组患者子宫内膜厚度及大小、月经量及不良反应明显少于对照组,差异有统计学意义（P〈0.05）.结论 采用宫腔内放置LNG-IUS治疗子宫腺肌病较口服孕三烯酮能更有效改善患者临床症状,作用时间长,且不良反应少,安全可靠,值得临床推广采用.Objective The aim of the study is to compare the therapeutic effects of levonorgestrel-releasing intrauterine system （LNG-IUS） and gestrinone in the treatment of adenomyosis. Methods We collected 120 patients with adenomyosis from Renming Hospital, Maoming, from January, 2014 to January, 2016, and randomly divided them into two groups： observation group （n=60） and control group （n=60）. The patients in control group were treated with gestrinone, and the patients in observation group were treated with LNG-IUS. The dysmenorrhea degree, uterine size, menstrual volume, and adverse effect were compared in pre- treatment, three months, 12 months after treatment, respectively. Results The dysmenorrhea degree, endometrial thickness and size, and menstrual volume after the treatment in two groups were significantly improved than pre-treatment （P〈0.05）. The dysmenorrhea degree in observation group was significantly lower than control group at 3 months and 12 months after treatment （P〈 0.05）. The endometrial thickness and size, menstrual volume, and adverse effects were significantly less than control group （P 〈 0.05）. Conclusion Compared to gestrinone, the adption of LNG-IUS to treat adenomyosis could improved clinical symptom better. It is with slight adverse reaction, safe, and reliable which could be spread in clinical application.
