摘 要:目的:探讨三种不同脐带结扎延迟时间对新生儿健康状况的影响。方法:随机选取我院产科符合纳入要求的顺产新生儿400例(2016年2月至2017年2月)进行研究,采用随机数字表法分为4组,其中100例采用延迟1 min后进行脐带结扎(A组)、100例采用延迟2 min后进行脐带结扎(B组),100例采用延迟3 min后进行脐带结扎(C组)、100例于出生后10 s内完成脐带结扎(D组),对比四组新生儿分娩24 h后的血常规、血清免疫球蛋白、分娩后不同时间血清胆红素、人氨基端前心钠肽(NT-pro ANP)、肌酸激酶同工酶(CK-MB)的水平。结果:C组新生儿的血红蛋白、红细胞比容、Ig G水平高于A、B、D组,差异具有统计学意义(P<0.05);A、B组新生儿的血红蛋白、红细胞比容、Ig G水平高于D组,差异具有统计学意义(P<0.05)。A、B、C、D四组新生儿的血清Ig A、Ig M水平差异无统计学意义(P>0.05);产后24、72 h,四组新生儿的血清胆红素、NT-pro ANP、CK-MB的水平比较差异均无统计学意义(P>0.05)。结论:延迟3 min进行脐带结扎方式有利于保证新生儿的血容量、提高免疫水平,同时并不增加新生儿的心脏负荷。Objective: To investigate the effect of three different types of umbilical cord ligation on neonatal health. Methods: A retrospective study was conducted on 400 newborn babies (from February 2016 to February 2017). There are 100 cases with delayed 1 min after umbilical cord ligation (A group), 100 cases with delayed 2min after umbilical cord ligation (B group), 100 cases with delayed 3 min after umbilical cord ligation (C group), 100 cases in 10s after birth in complete umbilical cord ligation (D group), compared four groups of neonatal childbirth after the 24 h blood test, serum immunoglobulin and serum bilirubin in different time after delivery, one N-terminal pro atrial natriuretic peptide (NT-proANP), creatine kinase (CK-MB) level. Results: The hemoglobin, hematocrit and IgG lev- els in C group were higher than those in group A, group B and group D (P〈0.05); The hemoglobin, hematocrit and IgG levels of the neonates in group A and B were higher than those in group D (P〈0.05), The levels of serum IgA and IgM in the four groups of A, B, C and D were not statistically significant(P〉0.05); The levels of serum bilirubin, NT-proANP and CK-MB in the four groups of postpartum 24 h, A, B, C and D were not statistically significant (P〉0.05). There was no significant difference in serum bilirubin, NT-proANP and CK-MB levels between the four groups after 72 h (P〉0.05). Conclusion: Delayed 3rain in umbilical cord ligation is helpful to ensure the neonatal blood volume, improve immune level, and does not increase neonatal cardiac load.
