改良四夹板外固定治疗儿童Ⅲ型孟氏骨折的疗效观察

摘　　要：目的常规正骨手法整复后,观察用改良四小夹板外固定法保守治疗儿童Ⅲ型孟氏骨折的疗效,并探讨分析其作用机制。方法对纳入治疗范围的62例儿童Ⅲ型孟氏骨折进行手法整复,采用改良小夹板超肘、腕关节外固定,指导功能活动锻炼,定期复诊及复查X线片,观察疗效。结果 62例经随访6~12个月(平均9个月),优50例,良9例,可3例,劣0例;优良率为95.2%,有效率100%。结论手法复位后用改良四小夹板超肘、腕关节外固定法治疗儿童Ⅲ型孟氏骨折的临床疗效肯定,值得临床推广,但其作用机制有待进一步探讨分析。Objective To observe the conservative treatment of Type m Monteggia fracture in children which use four small splint, and explore the mechanism, after manipulative reduction. Methods 62 cases of children with Type Ⅲ Monteggia fracture were collected which were treated with manipulative according to type of manipulative reduction with small splint elbow, wrist external fixation were included. Results 62 cases were followed for 6 - 12 months （ average 9 months） , 50 cases were excellent, 9 cases good, 3 cases of good, 0 cases of difference; The excellent and good rate was 95.2% , and the effective rate was 100%. Conclusion Traditional manual reduction with improved small splint elbow, wrist external fix- ation in the treatment of Type Ⅲ Monteggia fracture in children, the outstanding advantages, good functional recovery, but still need deep investigation.
