制定《中医治未病实践指南》的方法学探讨

摘　　要：目前中医文献评价对于现代临床研究性文献、现代临床观察性文献过于重视,而古代文献和现代专家的经验总结则被放入较低的证据级别的现象,建立具有中医特色的证据分级体系,其关键是通过对古代文献和现代专家的经验总结两方面的文献检索,然后对其进行评价和分级,最终将高质量的古今医学专家经验融入指南,从而使古今医学专家经验在循证中医临床实践指南中得到更好的体现,更好地指导临床。中医治未病虽然历代众多医家有所提及,但很少有中医治未病的专科论著,所以中医治未病的标准化在中医药标准化中是一个难点。本文在《中医治未病实践指南》化的过程中尝试以古典医籍文献为基础并对古典医籍文献进行分级,以GRADE评价为参考、以专家经验为特色、以专家共识为依据,综合考量。既保留了循证医学的研究方法,也考虑了中医学的特殊性,保留了中医临床的特点。相信通过对中医治未病证据搜集评价的研究探索,也能对中医药标准化提供借鉴。At present, the literature evaluation of Chinese medicine overstresses modem clinical research literatures and modem clinical observation literatures. The ancient literatures and experience of modern experts are put into relatively evidence level. The establishment of evidence grading system of Chinese medicine feature lies in the document retrieval of mlcient literature mid expe- rience of modenl experts. And then evaluate, grade them and include the experience of modern and ancient doctors into the guide- line for better guiding clinical practice. Even though doctors of multiple dynasties have mentioned preventive treatment of Chinese medicine, there are few treatise and monographs on it, which makes the standardization of preventive treatment of Chinese medi- cine difficuh in the standardization of＇ Chinese medicine. This paper tries to base on and grade classic medical literatures in the process of guideline development of the preventive treatment of Chinese medicine, take GRADE evaluation as reference, experts＇ ex- perience as characteristics and consensus of experts as evidences for comprehensive consideration. It maintains the research meth- od of evidence-based medicine and also takes the specificity of Chinese medicine into consideration. The exploration of the collec- tion and evaluation of the evidences of the preventive treatment of Chinese medicine will provide preferences for the standardiza- tion of Chinese medicine.
