益肺增效汤配合NP方案治疗晚期肺癌临床研究

摘　　要：目的:探讨益肺增效汤配合NP方案治疗晚期肺癌的临床疗效,评价其安全性。方法:选取86例晚期非小细胞肺癌患者作为研究对象,采用数字随机表法分为对照组和观察组,对照组给予NP方案治疗,观察组在对照组基础上给予益肺增效汤治疗,观察两组患者临床疗效、卡氏评分及中医证候评分,比较T淋巴细胞亚群变化情况及Ⅱ~Ⅳ级化疗不良反应。结果:观察组治疗有效率为83.72%,对照组有效率为62.79%,两组比较,差异有统计学意义(P<0.05);观察组治疗后卡氏评分为(87.36±7.11)分,中医证候评分为(7.21±1.58)分,对照组治疗后卡氏评分为(75.36±6.98)分,中医证候评分为(11.38±2.27),分,两组比较,差异有统计学意义(P<0.05);观察组治疗后CD+3、CD+4、CD+4/CD+8分别为(64.75±5.29)%、(37.02±5.31)%,(1.62±0.57)%,显著高于对照组,差异有统计学意义(P<0.05);观察组Ⅱ~Ⅳ级白细胞减少、消化道反应发生率分别为2.33%、6.98%,对照组分别为13.95%、23.26%,两组比较,差异有统计学意义(P<0.05)。结论:益肺增效汤配合NP方案治疗晚期肺癌能够减轻化疗不良反应,改善免疫功能,提高临床疗效。Objective： To investigate the efficacy and safety of Yifei Zengxiao Tang combined with navelbine and cisplatin（ NP）chemotherapy in the treatment of advanced lung cancer. Methods： 86 patients with advanced non small cell lung cancer in the hospital were selected as the research objects. The patients were divided into the control group and the observation group by using random number table method. The control group was treated with NP chemotherapy,while the observation group was treated with Yifei Zengxiao Tang,based on the treatment of the control group. The clinical efficacy and adverse drug reactions in the two groups were observed. The Karnofsky performance score（ KPS） and TCM syndrome score were performed before and after the treatment; The changes of lymphocyte subsets before and after the treatment were compared between the 2 groups. Results： The effective rate of the observation group（ 83. 72%） was significantly higher than that of the control group（ 62. 79%）（ P〈0. 05）; After the treatment,KPS score of the observation group（ 87. 36 ± 7. 11） was significantly higher than that of the control group,and the total score of TCM symptoms（ 7. 21 ± 1. 58） was significantly lower than that of the control group（ P〈0. 05）; The CD＋3（ 64. 75 ± 5. 29） %,CD＋4（ 37. 02 ± 5. 31） % and CD＋4/CD＋8（ 1. 62 ± 0. 57） % in the observation group after the treatment were significantly higher than those in the control group（ P〈0. 05）; In the observation group,the incidence rates of Ⅱ-Ⅳ grade hypoleucocytosis and gastrointestinal reactions（ 2. 33% and 6. 98%） were significantly lower than those in the control group（ 13. 95% and 23. 26%）（ P〈0. 05）. Conclusion： Yifei Zengxiao Tang combined with NP chemotherapy in the treatment of advanced lung cancer can reduce the side effects of chemotherapy,improve immune function and improve clinical efficacy.
