金胆片对胆囊癌术后患者感染情况及免疫功能的影响

摘　　要：目的:探讨金胆片预防胆囊癌术后感染的作用及对免疫功能的影响。方法:选择进行胆囊手术的患者100例作为研究对象,随机分成对照组和观察组,各50例。对照组患者于手术前4 d和术后8 d注射葡萄糖溶液,术后接受抗感染治疗,积极预防并发症的发生。观察组患者在对照组治疗的基础上于手术前4 d和术后8 d口服金胆片。观察治疗后两组患者肿瘤坏死因子(tumor necrosis factor,TNF-α)、白细胞介素-2(interleukin-2,IL-2)、IL-6、IL-10、内毒素、T淋巴细胞亚群及自然杀伤细胞(natural killer cell,NK)水平,比较感染情况。结果:术后7 d,观察组患者TNF-α、IL-6及内毒素水平分别为(46.5±14.2)ng·L-1、(43.2±14.5)ng·L-1、(25.4±7.1)ng·L-1,对照组患者TNF-α、IL-6及内毒素水平分别为(58.2±16.4)ng·L-1、(55.2±17.3)ng·L-1、(46.7±17.2)ng·L-1,观察组均低于对照组;且观察组患者IL-2、IL-10水平均高于对照组,差异有统计学意义(P<0.01)。术后4 d、7 d,观察组患者CD3+、CD4+及NK细胞水平高于对照组,CD8+、CD4+/CD8+水平低于对照组,两组比较,差异有统计学意义(P<0.01)。观察组患者的感染率为10.00%,对照组患者的感染率为26.00%,两组比较,差异有统计学意义(P<0.05)。结论:金胆片应用于胆囊癌围手术期能够减轻患者的炎症反应,提高患者的免疫功能,降低感染的发生率。Objective： To explore the effect of Jindan Pian on the prevention of postoperative infection and the immune function in patients who had undergone surgery for gallbladder carcinma. Methods： 100 patients were randomly divided into the control group and the observation group,with 50 cases in each group. Patients in the control group were given injection of glucose four days before and eight days after the operation,whereas those in the observation group were given oral administration of Jindan Pian and injection of glucose four days before and eight days after the surgery. Anti-infection therapy was utilized after the surgery to actively prevent the occurrence of complications. Results： Seven days after the surgery,the levels of TNF-α,IL-6 and endotoxin in the observation group were respectively（ 46. 5 ± 14. 2） ng·L-1,（ 43. 2 ± 14. 5） ng·L-1 and（ 25. 4 ± 7. 1） ng·L-1,all lower than that in the control group,which was respectively（ 58. 2 ± 16. 4） ng·L-1,（ 55. 2 ± 17. 3） ng·L-1 and（ 46. 7 ± 17. 2） ng·L-1,and the difference was statistically significant（ P〈0. 01）. Four and seven days after the surgery,the levels of CD＋＋3,CD4 and NK cells were all significantly higher than those in the control group,and the levels of CD＋4,NK and CD＋3 cells were significantly higher than those in the control group,and levels of CD＋8 and CD＋4/CD＋8 in the observation group were lower than those in the control group; the differences were all statistically significant（ P〈0. 01）. The infection rate in the observation group was10. 00%,lower than that in the control group,which was 26. 00%,and the difference was statistically significant（ P〈0. 05）.Conclusion： Intraoperative application of Jindan Pian for the surgery of gallbladder carcinoma can reduce the inflammatory reaction,improve the patients＇ immune function and reduce the incidence of infection in the patients.
