中药足浴治疗糖尿病周围神经病变用药规律研究

摘　　要：目的:探索中药足浴治疗糖尿病周围神经病变的中药、治法、足浴时间、联合用药情况,为临床应用提供理论依据和操作参考。方法:检索中国知网(CNKI)数据库中有关足浴治疗糖尿病周围神经病变的临床文献,对各文献中采取的中药、治法、足浴时间、联合用药情况利用Excel 2007和Cite Space软件进行统计分析。结果:纳入研究149篇文献。涉及用药140味,合计出现1 319次。其中药物频次≥20的有20味,合计出现910次,占总数的68.99%。使用频率较高的20味中药主要为活血化瘀药、祛风湿通经络药、温经散寒药、益气养阴药。中医治法出现频次≥10的主要有活血、通络、化瘀、止痛、益气、温经、养阴等。足浴时间为30min的文献89篇,占总数的59.7%,联合用药以控制血糖、营养神经、改善循环为主的西医治疗占总数的58.7%。结论:中药足浴治疗糖尿病周围神经病变主药虽以活血通络、化瘀止痛为大法,但应注意温阳、养阴、散寒等药物的配伍使用,该研究领域的严谨性有待提高。Objective： To explore the effects of factors on treating diabetic peripheral neuropathy with Chinese medicine foot bath, that factors including Chinese medicine, therapy methods, foot bath time and combined medication, so as to provide theoretical basis and operational reference for clinical application. Methods： This research took China National Knowledge Infrastructure （CNKI） as the database, retrieved the clinical papers related to treating diabetic peripheral neuropathy with foot bath, and to do statistical analysis on factors mentioned above using Excel 2007 and CiteSpace. Results： Totally 149 papers were included. There were 140 kinds of Chinese medical herbs and 1 319 frequencies involved. There were 20 kinds of Chinese medical herbs which were more than 20 frequencies, 910 frequencies in total and accounting for 68.99%. The 20 kinds of Chinese medicine were used frequently included theses, Chinese medicine of activating blood circulation for removing blood stasis, Chinese medicine of dispelling wind, eliminating dampness and dredging channels, Chinese medicine of warming channels for dispelling cold, and Chinese medicine of benefiting qi and nourishing yin. Chinese medicine treatments which were more than 10 frequencies were mainly about promoting blood circulation, dredging channels, removing blood stasis, relieving pain, benefiting qi, warming channels, nourishing yin, etc. Foot bath time in 89 papers which was more than 30 minutes accounted for 59.7%. Combined medication using Western medicine treatment to control blood sugar, nutrition nerves and improve the circulation accounted for 58.7%. Conclusion： Although the major medicines for treating diabetic peripheral neuropathy with Chinese medicine foot bath are promoting blood circulation and dredging channels, removing blood stasis and relieving pain, but also that concerted application of warming yang, nourishing yin and emitting chill should be paid attention, and the rigor of the research field needs to be improved.
