顾植山从“少阳为枢”角度运用柴胡桂枝干姜汤治验体会

摘　　要：柴胡桂枝干姜汤具有调节阴阳、和畅枢机、清热泻火、温中散寒等功效,主治寒热错杂之证候。顾植山教授认为,人体阴阳是以"开阖枢"的动态形式存在的,从运气学角度综合考虑患者的患病时间、地域、气候环境等,基于"少阳为枢"的理论运用柴胡桂枝干姜汤匠心独具。文章通过顾植山教授运用柴胡桂枝干姜治疗临床瘿病、腰痛、癌症等寒热错杂病机相似的不同案例,探讨其重视调理"少阳为枢"的临床诊治思维特点。顾教授尊重经典原方,同时亦根据患者病证的寒热轻重程度而适当调整。Chaihu Guizhi Ganjiang Decoction has the functions of regulating yin, yang and pivot, clearing heat-fire, and warming spleen and stomach for dispelling cold, which mainly was used to treat the syndrome of intermingled heat and cold. Professor GU Zhi-shan thought that the yin and yang were in the state of ＇open-pivot-close＇. So, when using the Chaihu Guizhi Ganjiang Decoction from the perspective of five evolutive phases and six climatic factors to consider the patient＇s illness time, area, climate and environment based on the theory of ＇Shaoyang as the pivot＇, he often achieved good clinical efficiency. This article aimed at exploring the clinical diagnosis and treatmem characteristics of Shaoyang as pivot through analyzing the cases of Professor GU Zhi-shan using Chaihu Guizhi Ganjiang Decoction to treat simple goiter, frequent micturition and pancreatic cancer after surgery with the similar pathogenesis of intermingled heat and cold. Professor GU respects the classic original, but also adjusts the dose of the original prescription according to the patient＇ s disease.
