摘  要:目的 了解重庆市巴南区第二人民医院住院婴幼儿社区获得性肺炎痰培养的细菌分布及耐药情况,为临床合理选择抗菌药物提供依据.方法 对2014年6月1日~ 2016年5月31日在儿科住院的3岁以下婴幼儿社区获得性肺炎患儿的痰液标本1321份进行细菌学培养.结果 共检出248株病原菌,其中G+菌157株(63.30 % );G —菌91株(36.69% );排名前三位的G+菌分别为金黄色葡萄球菌(36.70% )、肺炎链球菌(15.32% )、溶血葡萄球菌(5.24% );排名前三位的G—菌分别为大肠埃希菌(14.52% )、肺炎克雷伯菌(6.45% )、铜绿假单胞菌(2.42% );G+菌对青霉素、红霉素、克林霉素及阿奇霉素大多耐药;G—杆菌对阿米卡星、亚胺培南、哌拉西林/他唑巴坦、头孢哌酮/舒巴坦均敏感.结论 本地区婴幼儿社区获得性肺炎的主要致病菌为G+菌,且对青霉素、红霉素、克林霉素及阿奇霉素等常用抗菌素存在不同程度的耐药性,治疗应根据痰培养及药敏试验的结果选择合适的抗生素.Objective To investigate the distribution and drug resistance of bacteria in the sputum culture of community acquired pneumonia in infants,and to provide evidence for the rational selection of antibiotics in clinic.Methods On June 2014 to 2016 May 31 in my courtyard department of pediatrics hospital of children under 3 years old in communities get pneumonia sputum specimens 1321 bacteriological culture and the Hefei Hengxing HX-21 identification of bacteria drug sensitivity analysis system for identifica-tion of bacteria and drug sensitivity test.Results A total of 248 strains of pathogens were detected,the detection rate was 18.77%. Among the 157 strains G+(63.30% g);91 strains (36.69% ).(4 6 hemolysis Westwood methicillin resistant coagulase negative staphylococci (CNS)1 strains (0.40% ),producing extended spectrum beta lactam enzyme producing strains of Escherichia coli strains,pneumonia Klebsiella pneumoniae strains,a total of 10 strains (4.03% ).The top three G + bacteria were golden yellow staphylococcus (36.70% ),Streptococcus pneumoniae 15.32% ),hemolytic staphylococci (5.24% ).The top three of the G-bacteria were Escherichia coli bacteria (14.52% ),pneumonia Klebsiella pneumoniae (6.45% ),Pseudomonas aeruginosa bacteria (2.42% ), gram positive bacteria to penicillin,erythromycin,clindamycin and azithromycin much resistance;G-bacilli to amikacin,imipenem, piperacillin,piperacillin /tazobactam tazobactam and Cefoperazone /sulbactam andTanzania are sensitive.Conclusion Infants with community-acquired pneumonia was the main pathogenic bacteria of G +bacteria in the hospital,and there are different degrees of resistance to penicillin,erythromycin,clindamycin and azithromycin antibiotic treatment should choose appropriate antibiotics ac-cording to sputum culture and drug susceptibility test results.
