摘 要:目的在产前筛查中应用孕早期胎儿颈透明层(nuchal translucency,NT)测量联合中孕血清AFP及h CG检测,分析上述检测方式在产前筛查中的作用。方法选择初次产检的单胎妊娠孕妇,统计数据共有1 000例,对所有初次产检的单胎妊娠孕妇在NT测量孕早期予以NT测量,以血清甲胎蛋白(alpha fetoprotein,AFP)及β-h CG检测测量孕中期。对单独NT检测、单独血清学检测和联合检测的结果予以对比分析。结果单项超声检测NT厚度、单项母体血清学检测之间对比,差异不具有统计学意义(P>0.05),但联合检测的阳性率与单项超声检测NT厚度、单项母体血清学检测之间对比,差异具有统计学意义(P<0.05);在筛查胎儿唐氏综合征的诊断中,联合检测与单项检查的数据对比,差异具有统计学意义(P<0.05)。结论在孕早期予以NT测量,以血清AFP及β-h CG检测测量孕中期能够有效地提高唐氏综合征的检出率。Objective To detect the levels of serum AFP and hCG in pregnant women during the early stage of prenatal screening by nuchal translucency(NT), and to analyze the role of the above methods in prenatal screening. Methods A total of 1000 cases of singleton gravida pregnant women who had the first birth examination were selected. The data were collected from all the first trimester pregnant women. The NT levels were measured in the first trimester of pregnancy by NT, and the alpha fetoprotein(AFP) and β-hCG were used to detect the second trimester. The results of a single NT test, a single serological test and a combined test were compared and analyzed. Results The difference between the NT thicknessand the single maternal serological test was not statistically significant(P 〉0.05). However, the difference between the positive rate of the combined detection and the NT thickness and the single maternal serological test was statistically significant(P〈 0.05). In the diagnosis of fetal Down's syndrome, the difference was statistically significant(P 〈0.05). Conclusion The detection rate of Down's syndrome can be effectively improved by measuring NT in the early pregnancy and measuring the middle pregnancy by serum AFP and β-hCG.
