医院信息系统下延续护理信息平台的构建及应用

摘　　要：目的在医院信息系统(hospital information system,HIS)下构建延续护理信息平台,并探讨其应用效果。方法 2015年1-2月,半结构式访谈南通大学附属医院16个专科护理小组组长,并采用问卷调查法调查了210名妇科病区出院患者,结合访谈和问卷结果综合形成初期的用户需求,再进行两轮专家咨询,形成延续护理信息系统功能需求。按功能分类集成系统模块,以此为基础进行编程。结果对于医护人员客户端,80.00%的医护人员回答操作便捷、90.00%的医护人员认为有益于工作、100.00%的医护人员回答系统稳定、安全、85.00%的医护人员满意系统各模块外观设计、30.00%的医护人员对医护客户端提出新的需求、170名的医护人员参与了系统运行体验测试,显示各功能模块符合用户需求;223名患者成功签约使用延续护理应用软件并获得从院内到院外的延伸服务。结论应用基于HIS系统的延续护理信息平台,建立以签约医院护士团队为基础的延续护理服务,可尽快满足患者日益增长的康复及慢性病患者健康管理需求。Objective To construct a nursing information platform based on hospital information system （HIS） for providing transitional nursing care to clients for their life time, and to discuss the application effect. Methods Semi-structured interview was applied on 16 specialist nursing team leaders. A question- naire survey was conducted on 210 discharged patients from department of gynaecology.Initial users＇ needs were analyzed based on the interviews and questionnaires.After two rounds experts＇ consultation,the func- tional requirements of information system for transitional care were finally formed. Clients ~ needs were classified by functions,and then programmed.Results There were 80% medical staff replied easy to oper- ate,90% considered it is beneficial to work, 100% replied the system was stable, safe, 85 % was satisfied with the design,30 % users had new needs proposed to medical staff, 170 test users participated in the run- ning test.The results indicated that the functional modules meet the clinical requirements. 223 patients suc- cessfully signed up to use transitional care software to obtain services from hospital to community and home. Conclusion The application of the intelligent nursing information platform based on HIS can estab- lish transitional care service system conducted by the contracted nurse team, and meet the ever increasing health management and rehabilitation needs of chronic disease patients.
