老年髋部骨折患者术后居家护理及其需求的质性研究

摘　　要：目的了解老年髋部骨折患者术后居家的主要护理问题及护理需求的内容,为医护人员实施优质健康教育提供依据。方法 2016年12月,采用质性研究中的现象学研究方法,对在上海交通大学附属第六人民医院骨科进行髋部骨折术后1个月的患者进行半结构式深度访谈,并用现象学分析法进行资料分析。结果老年髋部骨折患者术后居家的主要护理问题有知识缺乏、心理焦虑;护理需求的内容有疾病的相关知识、功能锻炼的指导、换药拆线等。结论老年髋部骨折患者术后居家护理存在诸多的护理问题及需求,患者及家属希望多渠道得到相关知识,医院、社区应该高度重视此类患者,同时给予相应的指导和帮助,从而提高患者早期康复锻炼的依从性,尽早恢复日常生活自理能力。Objective To understand the main nursing problems and demands of elderly patients after hip fracture surgery,in order to provide the basis for medical staff to implement high quality health education. Methods By using phenomenological approach in qualitative research, semi-structured interviews were conducted among the elderly patients 1 month after hip fracture surgery in our hospital,and data were ana- lyzed by a phenomenological analysis.Results The main nursing problems of these patients were lack of knowledge and psychological anxiety. Nursing interventions should focus on health education, functional exercise instruction,wound care and stitch removal.Conclusion Elderly patients after hip fracture surgery have many problems in residential care. They need more channels to obtain knowledge. Medical personnels and community workers should attach great importance to these patients ,as well as give appropriate guid- ance and assistance, so as to improve their activities of daily living and their compliance to the functional re- habilitation training.
