摘  要:目的:探讨泵注生理盐水法在腔内心电图定位瓣膜式PICC中的应用效果。方法:对2017年4~5月我院34例需行PICC置管患者采用微量注射泵泵注生理盐水的方法打开三向瓣膜,引导腔内心电图进行导管尖端定位,观察置管准确性、腔内心电图稳定性、心电波形中断情况、生理盐水用量和操作用时。结果:34例患者中导管尖端到位准确率为94.1%,心电图稳定率为88.2%,心电波形中断率为23.5%,生理盐水平均用量为(4.5±2.6)ml,平均操作耗时(11.6±2.8)min。结论:泵注法可使PICC前端的三向瓣膜持续开放,引导出连续、稳定的腔内心电波形从而实现腔内心电图将瓣膜式PICC尖端放置在理想位置,生理盐水用量少,操作可控性好,简便实用,导管尖端到位准确性高。Objective: To explore the application effect of pump injection physiological saline method in localization of valve type PICC by intracavitary electrocardiogram. Methods: Three-way valves of a total of 34 patients requiring PICC catheterization in our hospital from April to May 2017 were opened by microinjection pump injection of normal saline to direct the endocardiogram and position the tip of the catheter. The intubation accuracy,stability of intracavitary electrocardiogram,interruption of electrocardiogram waveform,saline dosage and operation time were observed. Results: The accuracy rate of catheter tip in place among 34 patients was 94. 1%,the stability rate of electrocardiogram was 88. 2%,the interruption rate of electrocardiogram waveform was23. 5%,the average dosage of normal saline was( 4. 5 ± 2. 6) ml,and the average operation time was( 11. 6 ± 2. 8) min. Conclusion: The pump injection method can make the front end of the three-way valve open continuously to guide the continuous and stable electrical waveform in the cavity,so as to realize an ideal position of valve type PICC in the intracavitary electrocardiogram,with less dosage of saline,good controllability in operation,simple and practical,and high accuracy in catheter tip in place.
