负压封闭引流技术对创伤性骨折患者创面愈合及血浆CRP、WBC的影响

摘　　要：目的探讨负压封闭引流(VSD)技术对创伤性骨折患者创面愈合及血浆CRP、WBC的影响。方法将100例创伤性骨折患者随机分为VSD组和常规组,每组50例,常规组患者给予常规纱布换药处理,VSD组患者给予VSD技术处理。统计两组换敷料次数、抗菌药物使用时间、二期手术闭合创面时间、住院时间、创面感染率、多重耐药感染率;采用Kolcaba舒适状况量表(GCQ)评估两组患者舒适程度;检测两组患者术后血浆CRP、WBC水平。结果换敷料次数、抗菌药物使用时间、二期手术闭合创面时间、住院时间、创面感染率、多重耐药感染率VSD组明显少(低)于常规组,差异均有统计学意义(P<0.05)。治疗后1 d、二期手术闭合创面后1 d的GCQ量表评分VSD组明显高于常规组,差异均有统计学意义(P<0.05)。治疗后1 d、二期手术闭合创面后1 d的血浆CRP、WBC水平VSD组明显低于常规组,差异均有统计学意义(P<0.05)。结论VSD技术可减少创伤性骨折患者换敷料次数、抗菌药物使用和感染的发生,可促进创面愈合和改善患者的炎症状态,有利于提高患者的舒适度。Objective To investigate the influence of vacuum sealing drainage （ VSD ） technique on the effects of traumatic fracture patients with wound healing , plasma CRP and WBC levels.Methods Total 100 traumatic fracture patients were randomly divided into the conventional group and the VSD group , 50 cases in each group.The conventional group was given conventional gauze bandage treatment.The VSD group underwent VSD treatment.Statistical data included the number of times of dressing change , the duration of antibiotics application , the wound closure time of two-stage operation , hospitalization time , wound infection rate and the rate of multiple drug resistance infection.Kolcaba general comfort questionnaire （ GCQ） was adopted to assess patients＆#39; comfort level.Postoperative plasma CPR and WBC levels were also detected.Results All statistical data in the VSD group were all less （ lower ） than the data of the conventional group , there were significant differences （P〈0.05）.In the VSD group, the GCQ scores of receiving VSD treatment 1 d and the closure wound of two-stage operation at postoperative 1 d were higher than those in the conventional groups （P〈0.05）.The plasma CPR and WBC levels of receiving VSD treatment 1 d and the closure wound of two-stage operation at postoperative 1 d were also lower in the VSD group than the levels in the conventional group , there were significant differences （ P 〈0.05 ）.Conclusions VSD treatment can reduce the times of dressing change and the reliance of the antibiotics application , and the probability of infection , which is a method that can promote wound healing and ease inflammation ,and improve patients＆#39; comfort level effectively.
