摘  要:目的:研究1 550nm非剥脱性点阵激光(Non-ablative Fractional Photothermolysis,NFP)联合大光斑低能量1 064nm Q开关Nd:YAG激光(Q-Switch Nd:YAG,QSNY)治疗黄褐斑的有效性与安全性。方法:采用半脸、随机、研究者单盲的方法,将19例黄褐斑患者双侧面部均给予大光斑低能量QNSY激光治疗(每2周1次,共10次),之后随机选择一侧面部,再给予1 550nm NFP激光治疗(每6周1次,共4次),记录初始的黄褐斑皮损面积和严重度指数(melasma area severity index,MASI),治疗结束后4周与12周的MASI和医生主观评价(physician's global assessment,PGA),同时记录不良反应。结果:两组在治疗后4周和12周记录的MASI均有明显下降,均P<0.05。两组治疗后4周与12周的MASI和PGA比较,无统计学意义(均P>0.05)。两组均未出现严重不良反应。结论:目前没有证据表明,在大光斑低能量QNSY激光治疗黄褐斑的过程中,联合1 550nm NFP激光可以提高治疗效果。Objective To investigate the efficacy and safety of a nonablative 1550nm fractional photothermolysis(NFP)combined with a large spot-size,low-fluence Q-Switched neodymium-doped yttrium aluminum garnet(Nd:YAG)laser in treatment of melasma.Methods This is a split-face,randomized,observer-blinded study.In which 19 patients were treated with large spot-size,low-fluence QSNY for 10 sessions at 2-week intervals to the entire faces,and with the 1 550nm NFP for 4 sessions at 6-week intervals to the experimental face.Efficacy variables were Melasma Area and Severity Index(MASI),the physician&#39;s global assessment(PGA).Safety was evaluated through the reporting adverse events.Results The percentage of subjective improvement was virtually identical on both sides.Both in terms the degree of improvement and the lack of difference between the QNSY+NFP and QSNY treated sides.No serious side effects were reported in either side.Conclusion Our findings do not support the hypothesis of NFP providing a substantial benefit in treating the melasma when compared with the lone treatment of large spot-size,low-fluence 1064 QSNY.
