推广长处方对慢性病患者社区就诊意愿的影响分析

摘　　要：目的 :分析使用长处方对慢性病患者社区就诊意愿的影响。方法 :采用整群抽样的方法选取2016年8月马陆镇社区卫生服务中心沥苑家庭医生团队管理的慢性病患者1 200例为调查对象,通过征求患者同意后为其申请慢性病长处方,对研究对象使用长处方前、后在社区卫生服务中心和上级医疗机构就诊的情况进行统计分析。结果 :在申请长处方前,有843例患者在社区卫生服务中心进行慢性病随访和就诊;进行签约并申请长处方后,有1 023例患者在社区卫生服务中心进行慢性病随访和就诊。在社区卫生服务中心进行慢性病就诊的患者比例由70.3%提高到85.3%,差异有统计学意义(P<0.01),患者的就诊满意度达到98.3%。结论:推广长处方的使用有助于提升慢性病患者的社区就诊意愿,患者接受度和满意度较高,值得在社区卫生服务中心进行推广。Objective： To analyze the influence of extended prescription on the willingness of community visits of thepatients with chronic diseases. Methods： By using cluster sampling method 1 200 patients with chronic diseases managed by theLiyuan family doctor team of Malu Community Health Service Center were selected in August 2016 as the investigation objects.After consultation with the patients the extended prescription for chronic diseases was applied for them. Statistical analysis wascarried out on the patients who used the extended prescription before and after the study in the community health service centerand the higher level medical institutions. Results： Before applying for extended prescription, 843 patients were followed up andtreated in the community health service center for chronic diseases. After signing the contract and applying for the extendedprescription, 1 023 patients were followed up and treated in the community health service center. The proportion of the patientswith chronic diseases who visited in the community health service center increased from 70.3% to 85.3%, and the differencewas statistically significant （P〈0.01）. The patients’ satisfaction reached 98.3%. Conclusion： Promoting the use of extendedprescription helps to improve the willingness of the patients with chronic diseases to visit in the community, their acceptance andsatisfaction is high, and it is worth popularizing in the community health service center.
