摘 要:目的对温州地区无偿献血者血液标本进行乙型肝炎病毒血清学、核酸联合检测,全面评估两者在降低输血相关传染性疾病中的作用。方法采集无偿献血者血液标本60 758份,用2种不同厂家ELISA试剂检测乙肝表面抗原,2种试剂均阳性采用单人份NAT检测HBV-DNA,2种试剂均阴性或单试剂阳性进行6人份混样检测,混样阳性再进行单人份拆分检测,比较2种检测方法检出率的符合性。结果 60 758份无偿献血者血液标本中,血清学和核酸检测均阴性为60 505份,血清学与核酸检测均阳性为103份,血清学阳性核酸阴性为70份,血清学阴性核酸阳性为80份。HBs Ag双试剂阳性、单试剂阳性与核酸阳性符合率分别为79.3%、19.3%。结论血清学、核酸检测在输血相关传染病检测中是一种互补关系,任何一种方法减少均带来了血液漏检风险的增加。Objective The blood samples of unpaid blood donors in Wenzhou area were conducted for the combined serological and nucleic acid detection of hepatitis B virus,to evaluate the effects of both in the reduction of transfusion related infectious diseases. Methods A total of 60 758 blood specimens were collected from unpaid donors. Enzyme-linked immunosorbent assay( ELISA) agents from two different manufactures were adopted for the detection of hepatitis B surface antigen( HBs Ag). Specimens with positive results of both the reagents were conducted for the detection of HBV-DNA using single NAT,and specimens with negative results of the both reagents or single reagent positive were conducted for six mixed sample detection,and if the mixed samples detection was positive again,the specimens would be singled for split detection,and the compliance rate of the two detection methods was compared. Results Among 60 758 blood specimens,60 505 cases were both negative for serology and nucleic acid tests,103 were both positive for serological and nucleic acid tests,70 cases were serologically negative and 80 were serologically negative. The coincidence rates of HBs Ag double reagent positive,single reagent positive and nucleic acid positive were 79. 3%,19. 3%. Conclusion Serological and nucleic acid detection is a complementary relationship in detection of blood transfusion related infectious diseases,and reducing any one of them would increase the probability of missed detection.
