摘　　要：目的研究前B细胞克隆增强因子(PBEF)在不同中性粒细胞计数(ANC)的脓毒血症时的浓度变化及临床应用价值。方法选取125例成人脓毒血症患者作为研究对象,按白细胞计数水平进行分组;健康体检者30例作为对照组。收集所有患者的临床资料及血清标本,检测PBEF浓度,将患者各组PBEF水平与正常对照组进行比较。结果患者组中的中性粒细胞增高组、中性粒细胞正常组的PBEF水平与对照组进行比较,差异有统计学意义(P<0.05),中性粒细胞减少组、粒细胞缺乏组的PBEF水平与对照组进行比较,差异无统计学意义(P>0.05);脓毒血症组的PBEF水平与ANC计数有相关性(r=0.84,P<0.01)。结论 PBEF作为感染性指标应用于临床受外周血中性粒细胞数量影响,PBEF作为脓毒血症的辅助诊断指标具有局限性。Objective To investigate the level and clinical significance of serum pre-B-cell colony-enhancing factor（ PBEF）in sepsis patients with different neutrophil count levels. Methods 125 cases of adult patients with sepsis were selected,and grouped according to white blood cell count level; 30 cases of normal health examination were collected as the control group. Serum specimens and clinical data from all enrolled patients were collected. The concentration of PBEF was detected,and the levels of PBEF in each group were compared with those in the control group. Results The serum PBEF levels in increased neutrophils counts group and normal neutrophils counts group were compared with those of control group,and the differences were statistically significant（ P 〈 0. 05）. And the serum PBEF levels in neutropenia group and granulocyte deficiency group were compared with those of control group,but the differences were not statistically significant（ P 〈 0. 05）; PBEF level in sepsis had significant correlation with neutrophil count（ r = 0. 84,P 〉 0. 01）. Conclusion As an infectious indicator,PBEF is affected by neutrophils count of peripheral blood in clinical application,so PBEF has limitations as an adjunctive diagnostic criteria for sepsis.
