2015年西安市可疑肺结核患者痰涂片检查结果分析

摘　　要：目的分析西安市2015年可疑肺结核患者痰涂片检查情况,通过有效措施的改进促使痰涂片阳性检出率的提高。方法通过收集西安市2015年13个区(县)疾病预防控制中心结核病门诊登记的可疑肺结核患者痰涂片检查数据进行分析。结果 2015年共登记可疑肺结核患者查痰29 630份,检查痰涂片80 966张,阳性率为1.41%;初诊可疑者共查痰66 382份,痰标本合格率为68.87%。在不同性状的痰中,血性痰的阳性率最高,为15.06%,不同性状痰标本的涂片阳性率差异有统计学意义(χ~2=1 551.66,P<0.01);阳性率第四季度最高,为1.57%,不同季度阳性率差异有统计学意义(χ~2=8.89,P<0.05);不同地区痰涂片阳性率碑林区最高,为3.63%,不同地区痰涂片标本阳性率差异有统计学意义(χ~2=662.21,P<0.01)。结论西安市痰涂片阳性率较低;注意收集合格的痰标本;加强沣东区、高陵区和临潼区痰涂片检查质量。Objective To analyze the smear slides examination of tuberculosis（ TB） suspects in Xi＇ an,2015 and improve the smear positive rate through effevtive measures. Methods The data of TB suspects smear slides examination in smear slides examination registration book of outpatient departments from 13 county level Centers for Disease Control and Prevention（ CDC） was collected and analyzed. Results 29 630 TB suspects and 80 966 smear slides were registered and examined,and the positive detection rate was 1. 41%. 66 382 primary diagnosis suspects were examined and the smear quality qualified rate was 68. 87%.The positive smear slides check out rate of bloody smear was 15. 06% and the highest in the different quality smear samples,the differences in positive smear slides check out rates of different quality smear samples were statistically significant（ χ2= 1 551. 66,P〈 0. 01）; the highest positive smear slides check out rate was in the fourthquarter（ 1. 57%）,the differences in positive smear slides check out rate of different quarter showed statistical significance（ χ2= 8. 89,P〈 0. 05）; the positive slides rate（ 3. 63%）in Beilin district was the highest,and the differences in positive rates of slides in different regions showed statistical significance（ χ2= 662. 21,P〈 0. 01）. Conclusion The positive detection rate of slides in Xi＇an is relative low; we should pay more attention to collecting qualified smear samples,improving the inspection personnel operating skills and responsibility and strengthening the smear examination in Fengdong,Gaoling and Lintong Districts.
