摘 要:目的:通过病例对照研究,探索老年男性糖尿病下肢血管病变与血尿酸(UA)、C反应蛋白(CRP)相关性。方法:选择本院门诊或住院老年男性糖尿病患者200例,根据有无下肢血管病变分为有下肢血管病变(PVD)组(93例)和无下肢血管病变(NPVD)组(107例),测定两组患者CRP和UA水平。采用Pearson法分析CRP水平与UA的关系;采用Logistic回归分析下肢血管病变的危险因素。结果:PVD组患者血清CRP和UA水平均高于NPVD组,两组比较差异有统计学意义(P<0.05)。Pearson相关分析显示,CRP与SBP、DBP、BMI、TG、TC、LDL-C、SUA呈正相关,与HDL-C负相关,与Hb A1c、BUN、CR则无明显的相关性。Logistic回归分析显示,年龄、SBP、SUA、CRP、TG、LDL-C均是糖尿病患者合并下肢血管病变发病的独立危险因素。结论:炎性反应在糖尿病合并下肢血管病变的发生、发展中起重要的作用。血浆CRP和UA水平升高是发生2型糖尿病合并下肢血管病变的重要危险因素之一。Objective: Through case-control studies,to investigate the correlation between C-reactive protein( CRP) and blood uric acid( BUA),and lower limb vascular diseases in the elderly diabetic male patients. Methods: 200 elderly diabetic male patients in the clinics of our hospital were selected as the subjects,who were divided into PVD group( 93 cases) and NPVD group( 107 cases). CRP and UA level of both groups were determined. Then Pearson method was used to analyze the relationship between CRP and UA level,and logistic regression was used to analyze the risk factors causing lower limb vascular diseases. Results: The serum CRP and UA level in patients in the PVD group was higher than that of the NPVD group,which was of statistical difference( P〈0. 05). The correlation analysis of Pearson showed that CRP was positively correlated with SBP,DBP,BMI,TG,TC,LDL-C and SUA,but negative correlated with HDL-C. And it was not obviously correlated with Hb A1 c,BUN and CR. Logistic regression analysis indicated that age,SBP,SUA,CRP,TG,LDL-C were the independent risk factors for diabetic patients complicated with lower limb vascular disease. Conclusion: Inflammatory response plays a very important role in the generation and development of diabetic patients complicated with lower limb vascular disease. Increase level of serum CRP and UA is one of the risk factors causing type 2 diabetes complicated with lower limb vascular disease.
