丙型肝炎患者血清抗-HCV、HCV-RNA检测的临床意义

摘　　要：目的探讨丙型肝炎患者血清HCV-RNA定量与抗-HCV检测结果的关系及其临床意义。方法采用间接ELISA方法检测220例疑似丙肝患者的抗-HCV,采用实时荧光定量聚合酶链反应(FQ-PCR)技术检测其HCV-RNA水平。结果 220例抗-HCV阳性血清中,HCV-RNA阳性146例,占66.36%、阴性74例(占33.64%),另有5例抗-HCV阴性而HCV-RNA阳性,可能为HCV早期感染者;在不同抗-HCV定量值(≤20、20~、60~和≥100)患者中,HCV-RNA阳性率分别为:42.86%、67.37%、83.33%和84.00%,结果随着患者血清抗-HCV含量的增大,HCV-RNA的检出率亦相应增加(P<0.05)。结论抗-HCV检测仍是目前用于HCV诊断的主要检测手段,HCV-RNA载量检测对丙型肝炎的诊断和疗效判定具有十分重要的应用价值。所以运用HCV-RNA,联合抗-HCV检测,对正确诊断HCV及抗病毒治疗疗效的评估等都有重要意义。Objective To investigate the clinical significance and the relationship between detections of serum HCV-RNA and anti-HCV in patients with hepatitis C. Methods Anti-HCV was detected by indirect ELISA in 220 patients with suspected HCV infection,and the level of HCV-RNA of these subjects was also detected by real-time quantitative polymerase chain reaction（ FQ-PCR）. Results There were 146 cases with HCV-RNA positive in 220 cases of anti-HCV positive serum,accounting for 66. 36%,negative in 74 cases（ accounting for 33.64%）. There were 5 cases with anti-HCV negative but HCV-RNA positive,maybe due to early infection of HCV. The positive rate of HCV-RNA was different from patients with different anti-HCV quantitative value（ ≤20,20 ～,60 ～ and ≥100）,42. 86%,67. 37%,83. 33% and 84%. HCV-RNA quantity was increased with the elevation of the serum anti-HCV in patients infected with,HCV（ P 〈0. 05）. Conclusion Anti-HCV test is still used for the main detection methods for the diagnosis of HCV infection; HCV RNA quantity detection has great application value on the diagnosis and treatment of hepatitis C. So the combined use of HCV-RNA and anti-HCV detection is important for correct diagnosis and curative effect evaluation for hepatitis C.
