依托咪酯在老年患者无痛胃镜检查中的应用效果观察

摘　　要：目的研究依托咪酯在老年患者无痛胃镜检查中的应用效果。方法选取2016年2月至2017年2月于本院行无痛胃镜检查的130例老年患者为研究对象,采用电脑随机抽取分组方法将其分为对照组和观察组,每组各65例。观察组患者行依托咪酯+芬太尼复合麻醉,对照组患者行异丙酚+芬太尼复合麻醉。监测并比较两级患者无痛胃镜检查前和检查中各项指标、麻醉诱导时间、手术时间、苏醒时间、离室时间及改良警觉-镇静评分。结果无痛胃镜检查中,观察组患者收缩压、心率及呼吸频率均显著高于对照组(P<0.05),舒张压显著低于对照组(P<0.05)。观察组患者麻醉诱导时间和苏醒时间均显著短于对照组(P<0.05),离室时间显著长于对照组(P<0.05),改良警觉-镇静评分情况显著优于对照组(P<0.05)。结论依托咪酯能够稳定行无痛胃镜检查老年患者的收缩压、舒张压、心率及呼吸频率,提升麻醉诱导的安全性,且麻醉效果明显。Objective To study the effect of Etomidate used on painless gastroscopy in elderly patients. Method 130 cases of elderly patients with painless gastroscopy examination in our hospital from February 2016 to February 2017 were selected as the research objects, they were divided into control group（n = 65） and observation group（n = 65） by the method of computer random extraction, observation group patients received Etomidate combined with Fentanyl anesthesia, control group patients received Propofol combined Fentanyl anesthesia. Result During the painless gastroscopy examination, the systolic pressure heart rate and respiratory rate of observation group were significantly higher than those of control group（P〈0.05）, and diastolic pressure of observation group were significantly lower than those of control group（P〈0.05）, the anesthesia induction time and the awakening time of observation group were significantly less than those of control group（P〈0.05）, and the off room time was significantly longer than that of control group（P〈0.05）, the observes assessment of alertness/sedation scale score of observation group was better than control group（P〈0.05）. Conclusion Etomidate used in elderly patients with painless gastroscopy can stable systolic pressure, diastolic pressure, heart rate and respiratory frequency, improve the safety of anesthesia induction, and the effect of anaesthesia is obvious.
